It can be helpful to gather information about the supports you need when preparing for your NDIS planning meeting.

The following general information might be useful when preparing for your NDIS planning meeting.

  1. Have a think about what dietitian support (related to your disability) you’ll need over the next 12 months, or period of your plan.
    1. If you need Home Enteral Nutrition (HEN) or oral nutrition supplements, you should have ongoing dietitian support and advice to make sure you are getting the most from your nutrition.

      Nutrition care usually takes between 10 and 20 hours of dietitian funded time and 2 hours of allied health assistant funded time, per year.

      At your first appointment, a dietitian will work with you to develop a nutrition care plan and required hours to meet your goals.

    2. We provide health coaching and education to support people living with disability to have good nutrition and be a healthy weight.
    3. Identify any consumables you need to be built into your plan. This might include a gastrostomy feeding tube or giving sets and syringes for nutritional intake.
    4. Identify any thickeners, formula or nutritional supplements you need. If the cost of these are in excess of PBS funding and more than the usual cost of food, it should be built into your plan.
    5. If you require meal delivery services, you will need to identify the delivery costs so they can be included in your plan.
