Canberra Health Services Division of Medicine Inpatient wards provide care to patients across the division including: Gastro-medical; Respiratory / Cardiology/Endocrinology/Rheumatology; Infectious diseases/Neurology/Stroke; and Renal/General Medicine. The successful applicants will be expected to work Monday to Friday, business hours.

Duties

Specific responsibilities include, but are not limited to, the following.

  • Under direction of the Assistant Director of Nursing, the Discharge Liaison Nurse will:
  • In collaboration with the multi- disciplinary health care team assist in the coordination and delivery of discharge services to ensure they are patient centred, adhere to best practice and facilitate safe and timely transfer of care.
  • Develop networks with community services, professional and support groups including community referral options and provision of alternatives to inpatient care to provide the ongoing supportive care needs of patients/clients discharging from The Canberra Hospital.
  • Provide comprehensive patient care as part of the discharge process, including facilitation of communication with patients and carers and early escalation of identified issues utilising a risk management approach
  • Identify issues/concerns impacting on the early discharge of patients to the ward management team.
  • Comply with and deliver against the required key performance indicators in respect to Occupational Health and Safety, Workplace diversity.
  • Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Essential

  • Tertiary Qualification or related qualification
  • Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).

  • 3 years post graduate experience
  • Prior experience in medical wards

Appointment to this position is conditional on being granted and retaining appropriate clearances:

  • Undergo a pre-employment National Police Check.

Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates, and contingencies.

  1. Proven current expert knowledge in the application of discharge planning and proven ability to coordinate the discharge requirements of patients, particularly patients with complex care needs, utilising the nursing process to achieve an agreed standard of care.
  2. Demonstrated ability to work collaboratively as part of the multidisciplinary team to expedite the transition of patients/clients from hospital to home utilising a patient centred approach and screening process for at risk patients.
  3. Demonstrated highly developed written and oral communication skills, counselling and negotiation skills that support a patient’s transition to the community setting.
  4. Demonstrated experience and knowledge in a broad range of nursing medical conditions enabling proactive promotion of patient flow and patient /family education.
  5. Demonstrates understanding of, and adherence to, safety and quality standards, Work Health and Safety and the positive patient experience. Displays behaviour consistent with CHS’s values of reliable, progressive, respectful, and kind.
