Position overview
Under limited direction of the Link Nurse Manager you will oversee the after-hours community nursing service by coordinating and supporting the afterhours community nursing staff and workloads.
This part-time supervisory role is instrumental to triaging, attending rapid response calls, and providing effective support to patients in the community to enable them to remain in their own environment wherever possible. The role oversees interventions which actively contribute to the prevention of unnecessary presentations to acute facilities. Referrals are processed, when necessary, as an adjunct to the CHI service.
In addition, the RN3 acts as a resource clinician, providing advice to afterhours nursing staff, including the North Complex House AIN staff, as well as regional team community nursing staff on weekends and public holidays.
Key Responsibilities
Specific responsibilities include, but are not limited to, the following.
- Provide supervision to the Community Care Program after-hours nursing service, utilising leadership, coordination, and management skills; to provide high quality holistic care.
- Maintain and triage the patient intake services afterhours ensuring timely access to Community Care Program services for appropriate patients.
- Act as a professional role model. Be responsible for a delegated number of staff and provide guidance, support, direction, orientation and education to other staff and students.
- Provide direct clinical care. Maintain and promote safe clinical practice. Monitor clinical outcomes, measure the effectiveness of education and support provided to patients, carers, and family members.
- Actively participate in risk assessment, incident, and accident reporting, lead in continuous quality improvement activities, program-based research and accreditation that aim to improve the service to patients and the community nursing service.
- Maintain professional standards, current policies, and procedures. Participate in the credentialing processes as required by the department. Maintain clinical and other skills that are required for this position through accessing appropriate education and staff development opportunities. Enhance own professional competence through ongoing self-initiated research.
- Communicate effectively with patients and colleagues utilising well developed verbal, written and technological skills. Utilise computer programs, databases, and statistical information to assist in communication and to collect information for the generation of reports as required.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications and Experience
Essential
- Tertiary Qualification or related qualification in Nursing
- Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
Desirable
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
Experience
Essential
- A minimum of 5 years’ experience working professionally in a relevant nursing role.
Desirable
- Experience or knowledge of the CHS Exceptional Care Framework, Clinical Governance Framework, Partnering With Consumers Framework and all other related frameworks.

- Proven comprehensive range of advanced clinical skills and knowledge of evidenced based practice related to Community Nursing Practice. Familiar with the role of members of both allied health and health care agencies in the delivery of best practice health care.
- Highly developed organisational, leadership and problem-solving ability to be able to work as an independent and autonomous advanced clinical practitioner to ensure effective, efficient, appropriate decision making and coordination of health care delivery which meets patients’ needs in the afterhours environment.
- Demonstrated supervisory and delegation skills to appropriately match workloads with skill sets of staff on duty. Confidence to prioritise and triage high volume of incoming referral and requests that may have competing clinical needs.
- Demonstrated ability to operate computer systems and databases that are an integral part of patient management and monitoring of performance indicators relevant to the service delivery.
