Follow up
After your stay in hospital, we will advise you of your next treatment and oncologist appointment at the Canberra Region Cancer Centre.
If you give us permission, we will also send your GP a copy of your Discharge Summary. This letter provides the GP information about your stay in hospital, as well details of any follow up care you may need.
Discharge summary
A document that gives information on the medical care and treatment you received while in hospital, as well as what care, medicines and appointments you need after you leave.
It is important for your GP to know about your hospital stay so they can understand changes in your health and give you the best care.
Medications and pharmacy/scripts
If we have given you a script you will need to take it to your local pharmacy to be filled. Your GP may also give you scripts at your next GP appointment.
It is important to take the medications as prescribed.
If you have any questions about your medication, talk to your oncologist about your concerns.
Outpatient and community-based services
We have a number of outpatient and community-based services and clinics.
We provide services for you in your local community such as lymphoedema treatment, exercise physiology and cancer counselling. Did you know that you can pop into a Walk-In Centre to have your bandages or dressing changed?
We might refer you to one of our outpatient or community-based services after you leave hospital. Your treating team will let you know about this.
There are some services you can refer yourself to, or you can ask your GP or other health care worker to refer you. We have a list of all our services and clinics you can browse or search.
When completing your cancer treatment with us
Your oncologist will discuss your post-treatment follow-up schedule with you. We’ll arrange a follow-up appointment once you’ve finished your treatment.
Our Cancer Supportive Care Services Team can help with any community service referrals you might need after treatment.
Cancer Council ACT offers an Enriching Survivorship Program that you may find helpful after treatment.
Our Wellbeing Centre can help you with the transition to life after cancer treatment.
