The division of Cancer and Ambulatory Support provides a comprehensive range of cancer screening, assessment, diagnostic and treatment services and palliative care through inpatient, outpatient and community settings.
14A is 22 bed Haematology ward and 14B is a 28 bed Medical and Radiation Oncology, Rheumatology, Immunology and Dermatology ward incorporating all patients from diagnosis to end of life care.
We have a strong core staff and would like to expand this with committed, compassionate, and dedicated staff that can retain their humour and fun side in a very acute setting.
The Discharge Liaison Nurse is integral for supporting the flow and accessibility of the wards. The DLN liaises with the medical and allied health teams as well as external providers to support safe and timely discharges. This position is full time, Monday to Friday with no required shift work.
Duties
Specific responsibilities include, but are not limited to, the following.
- Working independently and in conjunction with other health professionals to lead and coordinate safe, appropriate, and timely discharge of patients from the CAS inpatient units to achieve targeted date of discharge
- Adopt an ‘early intervention’ approach and actively participate in family meetings to support patients and families through the discharge process ensuring patient centred care
- Coordinate multi-disciplinary and handover meetings to discuss management plan and discharge destination
- Co-ordinate, within tight time frames, the safe discharge/ transfer of patients from the unit on day of discharge including organising the transport and all relevant discharge documentation. Maintain effective communication with the multi-disciplinary team throughout this process to ensure hospital discharge times are met.
- Develop networks within community services, professional and support groups including community referral options and provision of alternatives to inpatient care to provide the ongoing supportive care needs of patients discharging from the Canberra hospital
- Provide comprehensive patient care as part of the discharge process, including facilitation of communication with patients and carers and early escalation of identified issues utilising a risk management approach
- Remain current in specialised knowledge, expertise and skills through continuing professional development and ongoing education.
- Identify issue/ concerns impacting the early discharge of patients to the ward management team
- Document and continually update discharge information including discharge dates and barriers
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications

Essential
- Tertiary qualification in nursing
- Registered or eligible for registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
Desirable
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
Experience
Specific responsibilities include but are not limited to the following.
Essential
- Minimum of 4 years post registration nursing experience
Desirable
- Prior experience in oncology / haematology will be highly desirable
- Understanding of post hospital community supports
- Fulfil the responsibilities of this role in alignment to the CHS Exceptional Care Framework, and all other related frameworks.
