Your treating team will work with you to discuss the most appropriate care following discharge. This may include a brief follow up with the Adolescent home intensive treatment team and referral to other community services, or this may be a referral to one of our community teams.

Access Mental Health Team
The Access Mental Health Team provide a 24-hour, seven day a week service. They assess and treat mentally ill people in crisis situations.
