Help with evidence for planning or change of circumstance review

If you are currently receiving NDIS funded supports through us and need some information to take to your next planning meeting, please let us know. You can:

request planning advice from your treating clinician or
call our NDIS Administration support team on (02) 5124 1067
email us at chs.racsndis@act.gov.au

If you want to use our services as part of your NDIS plan but are not currently a client, you can self-refer to our services by contacting the Central Health Intake line on (02) 5124 9977. If you are on a waiting list to be seen and have not yet been assessed, please refer to our general planning information for NDIS supports available through Canberra Health Services.
