When you apply for access to the NDIS, you may need to provide a report, assessment or letter from a relevant health professional as evidence of how your condition impacts your independence and need for supports.

If you are seeing a CHS health professional for care related to your disability support needs, you can ask them directly to provide you with the written information you need. They are unable to provide you with information regarding your disability needs if it is outside the scope of the care they are providing you.

If you are not currently seeing a CHS professional, we don’t accept referrals for the purpose of providing information for NDIS Access Requests.  Options for gathering evidence include talking to your GP, or other health care providers you may be seeing.  Alternately, if you need assistance to identify what evidence is required, you can contact the Local Area Coordinator (LAC) for assistance.
