The Primary Care Pilot (PCP) is a trial that aims to support people with complex needs to manage their health in the community so they are less likely to need to go to the emergency department (ED) or hospital.

About this service

Approximately 15 general practices are participating in the PCP. GPs from these practices will refer patients to the PCP if they are at risk of needing to go to ED or hospital.

The PCP 12-week program offers people at risk of going to hospital:

  • a care co-ordinator from Canberra Health Services
  • a small number of free GP appointments
  • a health care plan
  • linkage with services
  • health education and coaching
  • allied health services, if needed.

Who we care for

The PCP is for you if:

  • your GP is participating in the PCP,
  • you have complex health needs,
  • you have challenges accessing the health care you need to stay well at home, and
  • your GP thinks you are at risk of needing to go to ED or hospital.

The PCP offers you free health care while you are undertaking the 12-week program.

The PCP is a trial and that means you will need to complete a survey about the program. After the program has finished you may also be interviewed about the care you received and the experience you had with the PCP.

How to access this service

Your GP must refer you for this service.

What to expect

If you are referred to the PCP, your Canberra Health Services care navigator and GP will create your care plan with you so that you can receive the care you need.

Your care navigator and GP will help you better understand your health and the health system so you feel more confident managing your health once you complete the PCP program.

What to bring to your appointment

  • Medicare card
  • Any letters or correspondence from your GP
  • Medicines you need to take while you are here
  • List of medicines you are currently taking (or the boxes), including medicines you have brought without a prescription, such as herbal supplements and vitamins

Where we’re located

We will arrange to meet you at a location convenient to you such as a local Community Health Centre.
