If you give us permission, we will send your GP a copy of your Discharge Summary. This letter provides your GP information about your stay in hospital, as well details of any follow up care you may need.

It is important for your GP to know about your hospital stay so they can understand changes in your health and give you the best care.

Medications and pharmacy/scripts

If we have given you a script you will need to take it to your local pharmacy to be filled. Your GP may also give you scripts at your follow up appointment.

It is important to take the medications as prescribed.

If you have any questions about your medication, talk to your GP about your concerns.

Follow up appointments

In some cases you will need to make an appointment with your GP so they can refer you for scans or other tests. We will give your GP information about the care you received and any follow up care you may need in your Discharge Summary.

We may request an outpatient appointment for you in one of our specialist departments. You'll find out about your appointment once you're back home, by either a phone call or we'll send a letter.
