Our Diabetes Service provides care to help treat and educate adults and adolescents with all types of diabetes.

About this service

Our Service provides individual and multidisciplinary clinics from various locations across Canberra. Our team includes Endocrinologists, Diabetes Nurse Educators, Dieticians, Podiatrists, and a Social Worker.

Our services include:

  • Adolescent and young person service.
  • Continuous glucose monitoring.
  • Education program for people with cultural and/or special needs.
  • High risk foot service for people with or at high risk of acute foot complications.
  • Individual and group education programs for people with diabetes and their carers.
  • Insulin pump initiation and stabilisation.
  • Pregnancy service for women planning pregnancy or who have gestational, type 1 or type 2 diabetes during pregnancy.
  • Psychological support through diabetes social workers.
  • Specialised multidisciplinary medical, nursing and allied health services for adult clients in community health care settings.

Who we care for

We care for adolescents, adults (type 1 and complex type 2), as well as women during pregnancy.

We care for both inpatients (people admitted to hospital) and outpatients (people who attend the hospital or a clinic for an appointment or procedure).

Our services are available for adults and adolescents within the ACT and Southern NSW region.

We offer these services for free if you have a Medicare Card/ Pensioner or Asylum Seeker Card.

How to access this service

Your GP or medical practitioner will organise for you to access this service by providing a referral.

What to expect

We will contact you when your referral has been received and triaged.

Appointments are given to the most urgent cases first. We’ll contact you when an appointment is available.

During your appointment, one of our clinicians will ask detailed questions about your health problems and the medications (tablets/injections) you take. They will do a physical examination and they will probably order some tests (blood tests or scans) and follow up as needed. They may suggest changes to your tablets/injections.

Some appointments may be done by telephone or teleconference.

Our clinic will give you specialist information and advice. You will continue to have long-term care from your GP.

What to bring to your appointment

  • Medicare card
  • Any letters or correspondence from your GP
  • Health Care Card and/or concession card (if you have one)
  • Private health insurance card (if applicable/if you want to use it)
  • Medicines you need to take while you are here
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
  • Previous x-ray films, scans, ultrasounds or any other test results or reports
