Take Control – Live Well is a 3 week group program that gives you the skills and confidence to take control of your chronic condition. Connect with others and get support to make changes and live well.

About this service

Do you have a health condition that has lasted more than 3 months? Is it affecting your quality of life?

Take Control – Live Well is a FREE 3-week group program for adults in the ACT who have a chronic condition. It helps you find ways to reach your health goals and manage your condition. Connect with others in a safe and welcoming space.

You will learn how to:

  • get the best out of your health appointments.
  • look after your mental and physical health.
  • find reliable health information.
  • set and achieve your health goals.
  • find local support services.

After the 3-week program, you will have:

  • a plan to reach your health goals.
  • a program book of resources to take home.
  • the option to receive SMS support for up to 3 months.

Who we care for

Any adult (18+ years old) who lives in the ACT and has at least one chronic condition (lasting 3 or more months) can join the program.

What to expect

The program is run in two forms:

  • Online: Each session runs once per week for 2 hours. Carers are welcome to attend.
  • In person: Each session runs once per week for 2.5 hours. The program runs at various locations across Canberra. Carers are welcome to attend.

  • Phillip Community Health Centre - starting Tuesday 9 April 2024 at 9.30am
  • Online - starting Wednesday 8 May at 4.30pm
  • Gungahlin Community Health Centre - starting Friday 7 June at 9.30am.
  • Tuggeranong Community Health Centre - starting Wednesday 7 August at 1pm.
  • Belconnen Community Health Centre - starting Thursday 10 October at 1pm.

What to bring to your appointment

  • You do not need to bring anything.
