Rehabilitation at Home provides up to six weeks of home-based rehabilitation for people who need to improve their ability to complete activities in their home environment.

About this service

The Rehabilitation at Home team includes:

  • Physiotherapists
  • Occupational therapists
  • Allied Health Assistants.

We can visit you at your home for up to six weeks. We help you to do any exercises or practice daily activities to help you become as independent as possible in your home.

We will work on the things that you want to improve. This may include walking or doing other daily activities (for example dressing yourself or making a meal). We may help you to borrow equipment if you need it.

If you still need support or more rehab at the end of this period, we will help to transition you to services who can provide this.

Who we care for

We care for people who:

  • are 18 years or over and live in the ACT
  • have a recent injury or illness, which has led to a difficulty walking or doing other daily activities
  • need rehabilitation in their home environment. This includes people who are unable to attend rehabilitation appointments for reasons such as problems with walking, or being unable to use transport.

How to access this service

Your GP or other health professional can organise for us to see you by making a referral.

You can send your referral to the Central Health Intake.

What to expect

We will call you to arrange a time to visit.

During our first visit we will ask you what things you would like to achieve (your goals) during rehab. We will also complete assessments that tell us about your current ability and will usually include looking at you walking or doing activities that you want to improve. We will then discuss the plan for any further visits.

Most people will have visits twice a week, or more. This will depend on the type of activities you are undertaking in your rehab program and how you are managing at the time.

Part of your rehabilitation program will include a home exercise program that you can safely complete outside of our visits. The more of the program you can complete, the better your recovery is likely to be.

  • Medicare card
  • We will let you know if we need any medical letters or other information, to assist us to give you the best possible service.

Where we’re located

We provide mostly home-based therapy and may also include some appointments in other community locations. This will depend on your needs and your goals. We will always discuss your rehab program with you throughout your time with us.
