We help people with rehabilitation after a change in their ability to complete everyday activities because of a medical or health condition or disability.
About this service
Our goal is to assist you to maximise your independence, health and wellbeing. We do this by setting goals with you and implementing a therapy program.
The therapy program may include working with the following health professionals:
- Physiotherapy
- Occupational therapy
- Social work
- Speech therapy
- Psychology
- Neuropsychology
- Rehabilitation counsellors
- Allied Health assistants.
We will work together with you to achieve your goals and maximise your level of independence in areas that you would like to focus on.
We will then help you transition back to your life within your home, community, and workplace.
Who we care for
We care for people who:
- Live in the ACT (NSW residents who need multi-disciplinary rehabilitation may be eligible in some cases)
- Are aged 16 years or over
- have neurological conditions (such as spinal cord injury, stroke, multiple sclerosis, motor neurone disease, Parkinson’s disease) who have had a recent change in function
- People with non-neurological conditions who have experienced a functional change requiring rehabilitation.

How to access this service
Your GP or other health professional can organise for us to see you by making a referral.
You can also call Central Health Intake on (02) 5124 9977 to refer yourself.
What to expect
- We will support you to be at the centre of your rehabilitation.
- We will ask you to set goals, with support from our team, about what you would like to achieve while participating in rehabilitation.
- You will have appointments at the University of Canberra Hospital or in your home or the community if this is the best place for therapy to be undertaken.
- Our service is time limited; once you have achieved your goals, or your highest level of independence, we will assist you to transition back into your life with supports from your family, and any additional community supports you might need.
What to bring to your appointment
- Any letters or correspondence from your GP that relate to your current condition.
- We will let you know if we need any other medical letters or other information, to assist us to give you the best possible service.
