Position overview
The RADAR Team is a multi-disciplinary team that provides assessment and clinical management to older adults residing in the ACT. This team consists of Geriatricians, Advance Trainee Registrars, Registered Nurses, Occupational Therapist and Social Worker. Clients are referred by a number of areas both in the hospital and the community, including the GP’s, who have a deteriorating elderly patient who can potentially be managed at home, thus avoiding unnecessary hospital admissions. Clients recently discharged from Canberra Hospital are also referred to RADAR for a follow up if necessary.
The RADAR RN level 2 position works closely with Geriatricians, RN’s and the Allied Health team, and in conjunction with other health professionals to provide safe, appropriate, and timely care of patients in the community. The RN 2 will actively contribute to provision of short-term assessment and management of elderly clients who are living in the community in collaboration with other team members.
Key responsibilities
Specific responsibilities include, but are not limited to, the following.
- Provide an appropriate comprehensive health assessment that includes health history, physical examination, cultural, spiritual and social assessment to the elderly client group in the acute and community settings, while providing timely and accurate reporting of activities and performance.
- Assist in nursing expertise in clinical judgement by applying knowledge of common aetiology and clinical manifestations appropriate within the elderly client group in the acute and community settings.
- Ensure proven best practice principles are utilised and evidenced in the delivery of clinical services for the aged care client, by undertaking, participating, and assisting in research/quality assurance projects as required.
- Support the interprofessional model of education across the service providing education sessions within and outside the service as required.
- Identify and assist in developing and establishing policies and processes for safe practice, resource management and employee/industrial relations with RACS.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

- Able to work independently using advanced clinical knowledge of the elderly person
- Adaptability and flexibility to accommodate change and provide responsive services to meet clients’ needs
Qualifications
Specific qualifications include, but are not limited to, the following.
Essential
- Tertiary Qualification or related qualification in nursing
- Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
Desirable
- Relevant post graduate qualifications in nursing or gerontology
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
Experience
Specific qualifications include, but are not limited to, the following.
Essential
- Minimum 4 years’ full time post graduate work experience
- Driver’s License
Desirable
- Prior experience in gerontological nursing is highly desirable
- Experience or knowledge of the CHS Exceptional Care Framework, and all other related frameworks.
Prior to commencement
Appointment to this position is conditional on being granted and retaining appropriate clearances.
