Position overview
The Canberra Health Service (CHS) Diabetes Service is an interprofessional team comprising medical, nursing and allied health professionals. The service provides a coordinated and integrated service between the Canberra Hospital and various community-based locations within the ACT. Clients seen include adolescents and adults with diabetes and women with hyperglycaemia in pregnancy (HIP).
This nursing role requires the ability to provide appropriate skilled clinical assessment and intervention for clients with diabetes across CHS Health Campuses.
The Diabetes Nurse will be responsible professionally to the Senior Nurse Lead Diabetes.
Key responsibilities
Specific responsibilities include, but are not limited to, the following.
Under the direction of the Senior Nurse Lead of the Diabetes Services you will perform the role of a credentialled diabetes nurse educator.
- Provide a high level appropriate skilled nursing clinical assessment and intervention for clients with diabetes across CHS Diabetes Services.
- Contribute to the multidisciplinary team approach including the implementation of best practice and evidence-based models.
- Coordinate and participate in the development, delivery, monitoring and evaluation of quality diabetes nursing programs to clients at risk of and who have diabetes.
- Under professional supervision, participate in and/or implement and coordinate quality improvement programs, clinical policies, protocols and research programs.
- Be a role model, mentor, advocate, clinical teacher, resource and support person for diabetes nursing staff/students and other health professionals.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications
Specific responsibilities include, but are not limited to, the following.
Essential
- Tertiary Qualification in nursing
- Post-graduate qualifications from a university or tertiary institution in Diabetes Education. Is an Australian Diabetes Educators Association (ADEA) credentialed diabetes educator.
- Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
Desirable
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
Experience
Specific responsibilities include, but are not limited to, the following.
Essential
- A minimum 4 years’ work experience in a nursing
- Experience or knowledge of the CHS Exceptional Care Framework, Clinical Governance Framework, and all other related frameworks.
Desirable
- Prior experience in diabetes education and management will be highly desirable.
Prior to commencement
Appointment to this position is conditional on being granted and retaining appropriate clearances.
- Pre-employment National Police Check.

CHS is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates, and contingencies.

- Proven competence in developing, implementing and evaluating diabetes self-management education programs for people with diabetes of varying complexity (whilst utilising the nursing process) in a variety of inpatient and outpatient settings.
- Demonstrated problem solving skills and a proven ability to coordinate and support complex diabetes patient care within a multidisciplinary setting.
- Effective high level interpersonal and communication skills with demonstrated ability to liaise and foster positive relationships with all stakeholders in patient care, with a focus to achieve agreed health outcomes.
- Articulate and demonstrate capability to initiate, implement, embed and evaluate quality improvement, accreditation and research projects which contribute to improved knowledge, practice and care relevant to clients with diabetes.
- Demonstrates understanding of, and adherence to, safety and quality standards, Work Health and Safety and the positive patient experience. Displays behavior consistent with CHS’s values of reliable, progressive, respectful, and kind.
