Position overview

Women, Youth and Children Community Health Programs deliver a range of community-based primary health services to women, children, young people, and families. These services include support, education, and information; counselling, assessment and screening; early identification and referral; and the delivery of public health programs. Services are delivered within a multidisciplinary context if not by a multi-disciplinary team.

The Enhanced Health Services (EHS) includes the Early Family Support Service (EFSS), Enhanced Child Health Service (ECHS), Developmental Paediatric and Child Protection Medical Service (DPCPMS), and Women’s Health Service (WHS).

Early Family Support Service:

The EFSS delivers targeted services across the ACT for families with complex psychosocial needs who require additional support beyond universal Maternal and Child Health (MACH) services. The EFSS Nursing/Midwifery team works with parents and their newborn or young children within a sustained home visiting model and at outreach sites. The EFSS supports early identification and intervention for child development concerns, and information and support for parenting to build skills and confidence in the parenting role. EFSS Nurses/Midwives contribute to the health of families through preventive health initiatives such as breastfeeding promotion, immunisation programs and child safety support and through collaboration and strong multi-disciplinary work with the Early Family Support Counselling and Physiotherapy Team.

Key responsibilities

Specific responsibilities include, but are not limited to, the following.

Under direction of the Clinical Nurse Consultant, Early Family Support Service you will:

  • Provide high quality care, to clients experiencing an increased risk of poor health or developmental outcomes due to an intersection of complex health and psychosocial needs, that is consistent with current evidence and best practice whilst incorporating the vision and values of Canberra Health Services.
  • Effectively communicate, using a trauma informed lens, with consumers to accurately assess health needs and to plan, implement and evaluate interventions in accordance with existing practice standards, health promotion and illness/injury prevention strategies.
  • Work collaboratively with teams and services within EHS, broadly across WYC, and with external stakeholders and services, to support the delivery of timely, appropriate referrals and consultation, and holistic, comprehensive care that contributes to enhanced health and wellbeing outcomes for consumers.
  • Participate in the ongoing development, delivery and evaluation of education and training programs to staff, stakeholders, and consumers in the areas of child protection, family violence and other areas of clinical expertise.
  • Ensure a sound evidence base for clinical practice through the application of contemporary, best practice, interventions, and research in clinical nursing practice and in the development of policy, procedures, and quality improvement activities.
  • Actively contribute to education, support, and clinical supervision and of new nursing staff and students and model an exemplary level of professional and ethical standards in clinical interventions and upholding the National Quality and Safety Health Care Standards.
  • Contribute to a positive workplace culture by building collegial professional relationships and demonstrating a commitment to proactively seeking solutions when issues arise.
  • Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications

Specific responsibilities include, but are not limited to, the following.

Essential

  • Unconditional registration as a Registered nurse and/or midwife with relevant postgraduate qualifications in Child and Family Health.
  • Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Hold a current driver’s licence

  4. Demonstrated commitment to safe and evidence-based practice through participation and contribution to systems of Clinical Governance through the development of policy and procedure, research, and fostering a learning culture.
