Position overview

Rehabilitation, Aged and Community Services (RACS) is a vibrant and diverse division within CHS providing multidisciplinary rehabilitation, aged and community-based care across a range of settings. This includes Canberra Hospital, Community Health Centres and the Village Creek Centre in Kambah. RACS services work collaboratively with the individual, carers and other services within and external to CHS.

The Community Care Program (CCP) forms an integral part of the RACS division. The CCP is comprised of Nursing and Allied Health clinicians, who are supported by administrative staff and managers. Services on offer are either delivered in Community Health Centre clinics or within the client’s home, depending upon the nature of the service and the capacity of the client to travel.

The role of the CCP:

  • Work with people to gain the knowledge and skills to maximise self-management and promote independence.
  • Provide clinical care in the community to optimise health outcomes and avoid unnecessary hospitalisation.
  • Collaborate with general practitioners, primary care, and other care providers to meet the health and wellbeing goals of people in the community.
  • Work with people and carers to navigate their journey between home and health care settings.

Services provided include wound management, post-acute clinical care, CVAD management, continence education and management, stoma education, primary palliative care, and end of life care.

Key responsibilities

Specific responsibilities include, but are not limited to, the following.

  • Use advanced level knowledge, sound clinical decision making and critical thinking to provide holistic clinical care and case management. Provide leadership, direction and support in the team leader role.
  • Be accountable for the delivery of safe clinical practice and professional advice. Work collaboratively in leading the implementation and review of professional standards, policies and procedures. Fulfil the obligations of nursing professional codes of conduct and professional standards, while operating within legal, organisational, and legislative boundaries. Understand organisational strategies as they relate to your role and team goals; role model the CHS values, code of conduct and Employment Principles.
  • Understand and apply organisational obligations for risk identification, risk management and safe practice. Engage in incident reporting / investigation and develop risk minimisation strategies/ activities. Conduct quality improvement audits, lead quality improvement activities, program-based research, and accreditation activities.
  • Use communication channels effectively to liaise and develop strong relationships, display sound judgement and work collaboratively to create a positive practice environment that achieves best practice outcomes for recipients of the health care service.
  • Utilise departmental computer programs and databases to record and collect accurate and timely information for the generation of report and statistics.
  • Maintain own professional development portfolio and actively participate in promoting a learning culture by encouraging reflection, professional development, providing clinical teaching, in-service, and education. Participate in the preceptorship of new staff.
  • Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications

Specific responsibilities include, but are not limited to, the following.

Essential

  • Tertiary qualification or related qualification in nursing
  • Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Current driver’s licence

Desirable

  • Tertiary or post graduate qualifications in a wide range of clinical hospital and /or community health applicable to the position

Experience

Specific responsibilities include, but are not limited to, the following.

Essential

  • Minimum 4 years full time equivalent post graduate registered nursing experience or relevant post-graduate qualification

Appointment to this position is conditional on being granted and retaining appropriate clearances.

Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates, and contingencies.
