Position overview
The Cardiac and Heart Function Rehabilitation Program delivers care to patients with cardiovascular disease and heart failure in both inpatient and outpatient settings. Registered nurses play a central role, providing high-quality care through advanced assessment, education, support, and counselling for individuals and groups at risk of, or living with, these conditions.
The program operates in collaboration with a multidisciplinary team, drawing on a wide range of expertise to provide comprehensive, patient-centred care. It emphasises education on effective self-management strategies, underpinned by evidence-based practice. Services are provided during hospital admission, in the early post-discharge period, and throughout the patient's participation in Cardiac or Heart Function Rehabilitation.
The position is business hours, Monday to Friday, excluding public holidays.

Key responsibilities
Specific responsibilities include, but are not limited to, the following.
- Provide education to inpatients who have been admitted for medical, surgical or interventional management of a cardiac condition, tailoring information to patient needs.
- Undertake comprehensive clinical assessment of patients who have been referred to the cardiac and heart function rehabilitation programs, placing onward referrals to other relevant health professionals or programs.
- Provide leadership in co-ordinating and delivering safe and holistic care to patients participating in the cardiac and heart function programs.
- Participates in the development, reviewal and maintenance of clinical standards, policies and protocols, continuous quality improvement and clinical research.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications
Specific responsibilities include, but are not limited to, the following.
Essential
- Tertiary qualification in nursing
- Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a registered nurse.
Desirable
- A relevant post-graduate qualification would be highly desirable.
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
Experience
Specific responsibilities include, but are not limited to, the following.
Essential
- A minimum of four years full-time equivalent experience as a registered nurse.
- Comprehensive knowledge of cardiac anatomy and physiology.
- Experience or knowledge of the CHS Exceptional Care Framework, Clinical Governance Framework, and all other related frameworks.
Desirable
- Prior experience in cardiology or a secondary prevention program will be highly desirable

Appointment to this position is conditional on being granted and retaining appropriate clearances.

- Demonstrated leadership, advanced organisational and problem-solving abilities to ensure effective, efficient, and appropriate decision making in health care delivery.
- Demonstrated experience in delivering patient and staff education within the clinical environment, including cardiac risk factor modification and self-management strategies, with an ongoing commitment to fostering a learning culture through participation in quality improvement and evidence-based practice initiatives.
- Highly developed communication and interpersonal skills to work collaboratively within a multidisciplinary team, facilitating the coordination of complex care for patients and their families across diverse clinical and community settings.
- Demonstrates understanding of, and adherence to, safety and quality standards, work, health, and safety (WH&S) and positive patient experience. Displays behaviour consistent with CHS's values of reliable, progressive, respectful, and kind.
