Position overview

Hospital in the Home (HITH) is a unique Inpatient service of CHS.

HITH provides access to acute healthcare for patients, generally in their own home. The HITH nurses drive to patient’s homes to deliver clinical care. As well HITH nurses do ward rounds to coordinate patient referrals from wards to transfer patients to HITH and promote patient flow. HITH nurses also work on campus in the HITH unit. The Intravenous Access Team also sits under the HITH service.

The HITH service operates from 0730 to 2200, seven days a week. After 2200, a HITH nurse is “On-Call” for patient support.

Patients admitted to HITH are classified as “Inpatients” of CHS. They have been identified as medically stable and meet the criteria to be with the HITH service. Simply put, HITH is hospital substitution.

The Registered Nurse Level 2 (RNL2), in this role will plan and provide complex clinical care to adult patients, usually in their own home. This includes both working independently and as part of a comprehensive Multidisciplinary Team. The RNL2 will conduct advanced clinical assessments of patients, apply critical thinking skills, support and provide guidance to junior nursing staff, coordinate and troubleshooting complex care needs of patients within the HITH service. Any patient concerns will be escalated to medical staff as needed.

Key responsibilities

Specific responsibilities include, but are not limited to, the following.

  1. Provide client focused holistic care to CHS patients referred to the HITH service, using innovative, evidence-based nursing practice.
  2. Assess, plan, implement and evaluate nursing care for clients, including patient education on complex health needs. Work collaboratively with other members of the Multidisciplinary Team to achieve planned clinical patient outcomes.
  3. Provide education to patients, their families and other CHS staff on all aspects of the HITH service, participate in, contribute to, and deliver professional development programs.
  4. Provide leadership and supervision to new and less experienced staff as well as nursing students.
  5. Actively participate in and promote a culture of continuous quality improvement, policy development and clinical research that is evidence based in accordance with the CHS Quality and Clinical Governance Framework
  6. Maintain required clinical core competencies and skills, accessing appropriate education and staff development opportunities. Provide education and promotion of the HITH service across CHS.
  7. Participate and contribute to HITH data collection and Ket performance Indicators.
  8. Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications

  • Registered or eligible for registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Have a current Drivers Licence without restrictions

  • A minimum of 5 years relevant clinical nursing experience in acute medical and/or surgical nursing care of patients in an inpatient, ambulatory, or community care setting.
  • Prior CVAD knowledge and skills.

Appointment to this position is conditional on being granted and retaining appropriate clearances.

  • The successful applicant will be required to work morning and evening shifts on a 7-day roster that DOES include weekends and Public Holidays. As well, staff participate in an overnight “On call” roster, once identified suitable to do so.

Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates, and contingencies.
