Community Health and Integrated Care delivers a range of multidisciplinary, community-based services focused on prevention, early intervention, treatment, and ongoing management of health conditions. These services include Geriatrics, Rehabilitation, Nursing, Walk-in Centres, Breast Screen ACT, the Integrated Care Program, Renal Services, Bariatric Services, Women, Youth and Children, Community Paediatrics, the Spinal Pain Service, and components of the Chronic Pain Service, as well as services within the Rehabilitation and Aged Care Division. Services are delivered through a collaborative, patient-centred model of care that supports improved health outcomes for the ACT community and surrounding region.

The programs aim to foster a positive workplace culture and strong collaboration across clinical and operational teams to support the delivery of accessible and integrated community-based healthcare.

The Community Care Program, Community Nursing Service delivers a range of community-based nursing services to residents of the ACT. These services include direct nursing care in areas of wound, continence, stoma, post-acute support, palliative care, and the delivery of the self-management of chronic conditions program.

The “LINK” Nursing team provides after-hours support across Canberra Health Services, operating from 3pm–8:00am weekdays and 24 hours on weekends and public holidays. Based at The Canberra Hospital, LINK Nurses manage overflow from regional community nursing teams and respond to emergency (SOS) calls from appropriately identified CCP Community Nursing clients, Home Based Palliative Care clients and Cancer Rapid Assessment Unit patients. In addition, the team runs after-hours ambulatory care clinics (ACCs) at Weston Creek Community Health Centre and University of Canberra Hospital, open Monday–Friday, 4:30pm–9:00pm.

This position is the clinical lead on night duty and participates in a rotating roster that includes:
* Evening and night shifts on weekdays
* Morning, evening and night shifts on weekends and public holidays.

Key responsibilities

Specific responsibilities include, but are not limited to, the following.

  • Use advanced level knowledge, sound clinical decision making and critical thinking to provide holistic clinical care and case management. Provide leadership, direction and support in the team leader role.
  • Accountable for delivering safe clinical practice and professional advice. Collaborate in the implementation and review of professional standards, policies, and procedures. Maintain compliance with nursing standards, codes of conduct, and legislative requirements. Model CHS values, the Code of Conduct, and Employment Principles while contributing to organisational and team goals.
  • Understand and apply organisational obligations for risk identification, risk management and safe practice. Engage in incident reporting / investigation and develop risk minimisation strategies/ activities. Conduct quality improvement audits, lead quality improvement activities, program-based research, and accreditation activities.
  • Use communication channels effectively to liaise and develop strong relationships, display sound judgement and work collaboratively to create a positive practice environment that achieves best practice outcomes for recipients of the health care service.
  • Use departmental computer programs and databases to record and collect accurate and timely information for the generation of reports and statistics.
  • Maintain own professional development portfolio and actively participate in promoting a learning culture by encouraging reflection, professional development, providing clinical teaching, in-service, and education. Participate in the preceptorship of new staff.
  • Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications

Specific qualifications include, but are not limited to, the following

Essential

  • Tertiary qualification or related qualification in nursing
  • Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Current driver’s licence

Desirable

  • Tertiary or post graduate qualifications in a wide range of clinical hospital and /or community health applicable to the position

Experience

Specific experience includes but is not limited to the following.

Essential

  • Minimum 4 years full time equivalent post graduate registered nursing experience or relevant post-graduate qualification

Prior to commencement

Appointment to this position is conditional on being granted and retaining appropriate clearances.
