Community Health and Integrated Care delivers a range of multidisciplinary, community-based services focused on prevention, early intervention, treatment, and ongoing management of health conditions. These services include Geriatrics, Rehabilitation, Nursing, Walk-in Centres, Breast Screen ACT, the Integrated Care Program, Renal Services, Bariatric Services, Women, Youth and Children, Community Paediatrics, the Spinal Pain Service, and components of the Chronic Pain Service, as well as services within the Rehabilitation and Aged Care Division. Services are delivered through a collaborative, patient-centred model of care that supports improved health outcomes for the ACT community and surrounding region.

The programs aim to foster a positive workplace culture and strong collaboration across clinical and operational teams to support the delivery of accessible and integrated community-based healthcare.

The Community Care Program, Community Nursing Service delivers a range of community-based nursing services to residents of the ACT. These services include direct nursing care in areas of wound, continence, stoma, post-acute support, palliative care, and the delivery of the self-management of chronic conditions program.

The Complex Care CNC provides high-level clinical leadership within a community-based care setting, delivering expert support for the management of patients with complex subacute and chronic conditions. The role facilitates coordinated, person-centred care through collaboration with interprofessional teams, supports staff in managing complex clinical presentations, and promotes patient independence and self-management to enable people to remain living well in their home and community for as long as possible.

Key responsibilities

Specific responsibilities include, but are not limited to, the following.

  • Provide effective clinical leadership and consultancy to staff in the management of patients with complex care needs by working with stakeholders to develop comprehensive care plans. This includes but is not limited to: Identify and manage risks and issues, promote consumer engagement and participation, to empower patients to make informed decisions.
  • Provide clinical reports to management.
  • Review and provide consultation in the holistic management of patients with complex care needs, utilising an interprofessional model as clinically indicated.
  • Demonstrate commitment to patient advocacy, including the ability to support and empower patients and carers to effectively navigate the health system, access appropriate services, and achieve optimal health outcomes.
  • Provide active leadership in clinical handover discussions and promote a culture of shared learning and development within the workplace.
  • Contribute to the development, management, facilitation, and evaluation of educational programs.
  • Participate in and understand how risk management, quality improvement, nursing practice review and clinical research activities promote optimum service delivery.
  • Participate in forums and meetings within Community Care and represent Canberra Health Service as directed.
  • Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.
