Position overview

The Renal Network provides comprehensive care for patients with all stages of renal impairment across the ACT and Southern NSW. Services include inpatient and outpatient care such as acute haemodialysis, satellite and home dialysis, the Chronic Kidney Disease Program, Renal Supportive Care, Renal Transplantation, Dialysis Access, General Nephrology, clinical trials, and human research. The team adopts a multidisciplinary, evidence-based approach and is dedicated to delivering patient-centred care.

The Transplant Nurse will provide expert clinical advice to patients, carers, and other health care professionals within the defined specialty of Renal Transplantation. The Transplant nurse contributes to the development and delivery of speciality related education programs for both patients and staff around pre- and post-transplant care and support.

The Transplant Nurse works with other Transplant nurses to prepare patients across the Renal Network for Transplant Suitability and monitoring post-transplant closely with the Multidisciplinary teams within the ACT and Southern NSW Local Health District Renal Network and reports to the Clinical Nurse Consultant of Renal Outpatients CHS. The nurse also will be required to work the Transplant Facilities across Australia to coordinate patient care.

This position is part time across roster working Monday to Friday during normal business hours and is based at The Canberra Hospital.

Key responsibilities

Specific responsibilities include, but are not limited to, the following.

  • Provide comprehensive client focused care and education to patients referred to the Renal Services Transplant program and Renal Outpatient Service clinics. Use critical and analytical problem solving and decision-making skills whilst adhering to clinical policies and professional codes of practice. This includes an understanding and commitment to meeting the Australian National Standards for Quality and Safety in Healthcare.
  • Maintain own professional development activities and demonstrate relevant knowledge and understanding of current trends in renal nursing. Also participate in research, educational and quality initiatives undertaken by the multidisciplinary team, to improve patient care and service delivery.
  • Contribute to effective communication processes including working with challenging behaviours and resolution of conflicts.
  • Provide leadership and promote a commitment to high quality customer service principles and practices.
  • Precept and mentor junior nursing staff, assisting with training and assessing competency of new colleagues to attain consistency of nursing practice standards and local service requirements.
  • Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Registered or have applied for registration with the Australian Health Practitioner Regulation Agency (AHPRA)

Specific experience includes, but is not limited to, the following.

Essential

  • A minimum of four years’ full‑time equivalent experience working as a Registered Nurse
  • Renal Certificate or relevant experience working in Renal
