Rehabilitation, Aged and Community Services (RACS) is a vibrant and diverse Division within Canberra Health Services providing multidisciplinary rehab, aged and community-based care across a range of settings. This includes Canberra Hospital, University of Canberra Hospital, Community Health Centers and Village Creek Centre in Kambah.

The Rapid Assessment of Deteriorating Aged at Risk (RADAR) team is a multi-disciplinary service that provides assessment and clinical management to older adults residing in the ACT. This team consists of Geriatricians, Advance Trainee Registrars, Nurse Practitioner, Registered Nurses, Occupational Therapist and Social Worker. RADAR supports clients who are at risk of deterioration in the community, as well as inpatients referred prior to discharge, with the aim of preventing unnecessary hospital admissions and reducing likelihood of readmission.

RADAR Clinical Nurse Consultant (CNC) is responsible for the day-to-day operational management of RADAR services including the management of nursing workloads, the model of care, and patient flow. This role works closely with the nursing workforce and other health professionals to lead and coordinate safe, appropriate, and timely care of patients in the community. CNC provides expert clinical leadership and management within the nursing and multidisciplinary team and actively contributes to the short-term assessment and management of elderly clients living in the community. The CNC will also develop and maintain collaborative partnerships with internal and external stakeholders to facilitate timely and appropriate patient flow. is the role is expected to promote, incorporate and maintain the National Safety and Quality Health Standards

The successful applicant will be expected to maintain accountability in their own professional practice, engage in ongoing education and work collaboratively with the RACS division to support the professional development of the nursing workforce.

Key responsibilities

Specific responsibilities include, but are not limited to, the following.

  • Provide expert clinical and management leadership ensuring the safe, appropriate and person-centered delivery of care, along with high-quality customer service at an operational level.
  • Maintain CPD and evidence-based practice requirements for nursing registration for both self and team members. Remain current in specialised knowledge, expertise and skills through ongoing professional development and education. Ensure that RN within the team maintain clinically competence by providing appropriate support, guidance and education.
  • Lead and manage standards of care, Clinical Governance activities and Service Planning. Facilitate Quality Improvement (QI) initiatives and promote best practice to support successful accreditation against the National Safety and Quality Health Care Standards.
  • Be accountable to the ADON for optimal utilisation of resources, including responsible for rostering, leave management, recruitment, establishment and human resource matters such as staff performance and development.
  • Work collaboratively to plan and implement effective patient flow through intake, assessment, case management, discharge, and referral processes. This includes monitoring and management of clinicians’ workloads and availability to ensure delivery of best-practice care.
  • Prepare and analyse management reports, advising the ADON on activity, staffing, and related issues. Actively participate in responding to variations by monitoring key performance indicators (KPIs) and contributing to data review and analysis to support ongoing projects, RACS performance data integrity and quality activities.
  • Lead, coordinate and work in partnership with the multi-disciplinary team to facilitate change management initiatives aligned with the strategic objectives of Rehabilitation, Aged and Community Services, Canberra Health Services.
  • Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications

Specific responsibilities include, but are not limited to, the following.

Essential

  • Tertiary Qualification or related qualification in Nursing
  • Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Hold a current driver’s license

Desirable

  • Post Graduate studies in Nursing, Health Management or related field.
  • Hold Train the Assessor course
