Position overview
The Cardiac Rehabilitation Coordinator (CRC) is a pivotal role in the provision of secondary health prevention to patients who have suffered a cardiac event. This role involves working within prescribed legislation to provide proficient clinical, management, quality and safety, and educations leadership. The CRC will triage referrals and engage with stakeholders both internal and external, coordinate the Cardiac Rehabilitation Outpatient Program and provide education and support to staff and patients within the CCU. The Cardiac Rehabilitation Coordinator is the clinical leader of the multidisciplinary team which provides for the needs of individual patients and supporting them in their recovery and journey towards a healthier lifestyle.
Key responsibilities
Under limited direction of the Assistant Director of Nursing ICU/CCU Critical Care you will have the ability to work independently and part of a multidisciplinary team. You will:
- Provide holistic patient care by delivering advanced clinical assessment, planning, implementing, and monitoring of patient health care needs in accordance with existing practice standards and guidelines to achieve best practice health outcomes.
- Facilitation and delivery of patient education, clinical review and rehabilitation classes in conjunction with the multidisciplinary team in the outpatient setting accounting for diversity and different learning styles.
- Delivery of nursing education demonstrating a commitment to fostering the learning culture through active contribution to specialty education and training of self and others, knowledge of current guidelines.
- Participates in the development, review and maintenance of clinical standards, policies and protocols, continuous quality improvement and clinical research.
- Keeping program statistics- including: referral numbers, attendance rates, outcome data.
- Practice in accordance with relevant Government Health policies and procedures, the position description, Code of Conduct and industrial agreements and comply with relevant Canberra Health Service (CHS) WHS policies, procedures, work instructions and requests;
- Participate in recruitment/retention, selection, orientation processes and other human resources activities as per delegation.
- Patient satisfactions as measured by compliments, complaints and formal surveys
- Take a strong leadership role and implementing effective management practices in relation to Identifying areas for practice development and quality improvement, research opportunities and conducting and responding to audits relating to the NSQHS Standards.
- Demonstrating the principles of workplace equity, safety and injury prevention in work practices and interactions with patients, visitors, contractors and other CHS employees and clients and take reasonable care of your own health and safety and the health and safety of others in the workplace;
- Accountability for the management and reporting of risk by patients, visitors and contractors.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.
Competencies

Qualifications
Specific responsibilities include, but are not limited to, the following.
Essential
- Post Graduate Qualification in a relevant discipline.
- Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Advanced life Support.
- 100% compliance with Mandatory training on induction and maintained annually.
Desirable
- Qualifications in Cardiac Rehabilitation or Advance Practice Nursing.
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
Experience
Specific responsibilities include but are not limited to the following.
Essential
- Minium 5 years post registration experience working in an Acute Care Hospital.
- Prior experience in a Cardiology environment.
- Demonstrated experience working as an independent clinical practitioner.
Desirable
- Prior experience in Cardiac Rehabilitation or Heart Failure.
- Computer literacy skills and proficient use of CHS applications.
- Fulfil the responsibilities of this role in alignment to the CHS Exceptional Care Framework and all other related frameworks.
Prior to commencement
- Pre-employment National Police Check.
- Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.

- Demonstrated broad clinical and operational experience in the area of Cardiac Rehabilitation with demonstrated advanced, comprehensive nursing assessment skills, and the ability to plan and implement comprehensive care plans for the patient post cardiac event.
- Demonstrated proficient leadership, problem solving, organisational, and communication skills with the ability to liaise effectively with diverse internal and external stakeholders and interact as part of the multidisciplinary team including effective negotiation and conflict resolution.
- Demonstrated ability to utilise and participate in the development and presentation of adult education programs, as well as ability to prepares and deliver a range of learning and professional development opportunities for nurses who are providing inpatient education.
- Demonstrated experience in identifying practice development, audit and research opportunities in the Cardiac rehabilitation space as well as commitment to own professional development including affiliation with relevant professional organizations
- Demonstrates understanding of, and adherence to, safety and quality standards, Work Health and Safety and the positive patient experience. Displays behaviour consistent with CHS’s values of reliable, progressive, respectful, and kind.
