Position overview
Clare Holland House (CHH) provides palliative care including pain and symptom control, respite and end of life care as part of Canberra Health Services (CHS). We are focused on supporting patients and families who are dealing with life limiting illness. Our goal is to help patients achieve dignity, comfort and maximise the quality of life as they progress into end of life. Clare Holland House comprises of an inpatient unit, Specialist Palliative Aged Care Team (PEACE), and community specialist palliative care service (CSPCS), which provide palliative care to the Australian Capital Territory (ACT) and surrounding region.
Key responsibilities
Under limited direction of the team leader, you will:
- Work under direct and indirect guidance, supervision and support of Registered Nurses as a member of the nursing care team and contribute to and promote the physical, psychological, social, cultural and spiritual care of the patient.
- Provide safe and effective assistance to allocated patients and/or nursing teams to support specific needs in performing activities of daily living.
- Communicate with members of the nursing care team verbally and in writing through the patient’s clinical record and provide specific feedback to Registered Nurses on the personal care given.
- Contribute to an environment that is respectful for the personal dignity and integrity of the patient.
- Provide one on one care to patients who are at risk of harm to self (for example high falls risk, confusion) or others as requested by the Clinical Nurse Consultant or nursing Team Leader.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications
Specific responsibilities include, but are not limited to, the following.
Essential
- Relevant Certificate III in Health Services Assistance or recognised equivalent (for example working towards Diploma in Nursing or Bachelor of Nursing) is essential.
- The successful applicant will need to be available to work shifts 7 days a week, mornings, evenings and night shifts.
- Have an understanding of how the National Standards and Quality Health Service (NSQHS) indicators align with this role.
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
- Fulfil the responsibilities of this role in alignment to the CHS Exceptional Care Framework and all other related frameworks.
Experience
Specific responsibilities include but are not limited to the following.
Essential
- Several years’ work experience in a similar role and role environment
Desirable
- Prior experience in palliative care will be highly desirable
Prior to commencement
- Pre-employment National Police Check.
- Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.

CHS is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates, and contingencies.
