Duties

Under direction of the Clinical Nurse Consultants, Diabetes Service you will perform the role of a Registered Nurse, providing direct patient care, and involved in education, research, and improvements in service delivery. You will:

1. Provide patient centred, competent and safe nursing care, including the provision of self-management skills, to patients at risk of and who have diabetes.
2. Contributing to the multidisciplinary team approach including the implementation of best practice and evidence-based models.
3. Actively participating in attaining knowledge and skills relevant to practising as a diabetes nurse educator.
4. Fostering, promoting and providing a commitment to high quality customer service as outlined in customer service standards.
5. Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Position Requirements / Qualifications

• Relevant qualifications as a Registered Nurse with the Australian Health Practitioner Regulation Agency (APHRA)
• Working towards or commitment to undertake post graduate Australian Diabetes Educators Association (ADEA) accredited Graduate Certificate in Diabetes Education and Management. Is an Australian Diabetes Educators Association (ADEA) member and has or is working towards becoming a Credentialed Diabetes Educator.
• Must hold and maintain a current driver's license.
• Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
• Fulfil the responsibilities of this role in alignment to the CHS Exceptional Care Framework and all other related frameworks.
• CHS is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates and contingencies.

• Undergo a pre-employment National Police Check.
• Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.

What you Require (Key Selection Criteria)

These are the key selection criteria for how you will be assessed in conjunction with your resumé and experience.

1. Proven knowledge and skills to provide safe nursing practice in community and tertiary health care settings.
2. Demonstrated interest and knowledge in diabetes self-management education and commitment to undertake post graduate Australian Diabetes Educators Association (ADEA) accredited Graduate Certificate in Diabetes Education and Management.
3. Demonstrated ability to work collaboratively as part of a multidisciplinary team while providing high quality customer service.
4. Demonstrated highly effective communication (oral/written) and interpersonal skills.
5. Demonstrates understanding of, and adherence to, safety and quality standards, work, health and safety (WH&S) and the positive patient experience. Displays behaviour consistent with CHS’s values of reliable, progressive, respectful and kind.
