Position Overview

The ICU Liaison Nurse is a clinical expert who works from within the ICU Medical Team to follow up ICU patients once discharged to the ward, and who functions as a resource for medical, nursing and allied health staff working on the general wards, to assist in the management of patients with complex care needs or episodes of patient deterioration.

The ICU provides an ICLS nurse to support the wider hospital seven days a week for eight hours a day. This role aims to better manage the transition of patients from the ICU to the ward and to support the treatment of deteriorating or complex patients in the ward environment, avoiding unnecessary ICU admission, or facilitating their admission to ICU. In this way the ICU liaison nurse acts as a bridge between the skills and resources of the ICU and the ward environment, supporting continuity of care and patient flow. The role also includes MET data review, formal and informal education of ward staff and support for the families of critically ill patients, including end-of-life discussions and palliative care.

Key responsibilities

Specific responsibilities include, but are not limited to, the following:

Under direction of the ICU/CCU Assistant Director of Nursing (ADON) you will perform clinical and administrative duties in line with a RN L3.1 role.

  1. Providing expert clinical advice and assessment of patients following transfer from ICU or acute deterioration.
  2. Contribute to the development and maintenance of standard operating procedures, policy and procedure and other administrative and governance documents required for ICU liaison service as delegated.
  3. Provide leadership, support and direction to all staff, patients and families within the hospital ward environment, including a close relationship with the After-Hours manager for the Hospital.
  4. Apply clinical knowledge and experience in the clinical assessment and investigation of patients and collaborate with the treating team and the ICU multidisciplinary team in the clinical decision-making process where appropriate.
  5. Conduct immediate and ongoing assessment of referred patients including advanced physical assessment, medical / surgical / psychosocial history identification, recent and current treatment regimens, and document findings appropriately. Clinical review of individual patients including but not limited to:
    • Responding to requests for assistance in both the inpatient and outpatient areas.
    • Facilitation of critical care patient transfer to the ICU/CCU.
    • Attend MET calls initiated during the shift to provide clinical assistance/leadership as required.
    • Follow up assessment of MET calls where necessary, when ongoing treatment is maintained outside the intensive care/coronary care unit.
    • Follow up assessment of HDU and ICU patients transferred from the ICU to the ward areas.
    • Follow up assessment of identified unstable patients in the ward/ED.
    • Initiate clinical intervention within scope of practise and in conjunction with the medical emergency team (MET) Communicate specific assessment/treatment concerns to, and liaising with, relevant treatment teams.
    • Participate in the TPN round.
    • Coordinate and manage complex patients utilising evidence-based practice resulting in optimal patient outcomes.
  6. Provide clinical education to staff within and external to the Intensive Care Unit both formally and informally as required.
  7. Participate in data collection as delegated to improve service delivery via regular evaluation of outcomes.
  8. Participate in quality improvement, research projects and change management in relation to ICU liaison service.
  9. Contribute to the NSQHS Standard 8 committee (Recognising and Responding to Clinical Deterioration).
  10. Contribution and as required take leadership role in the MET M&M and associated reports.
  11. Manage direct reports of Co-worker ICU Liaison nurse team.
  12. Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Position Requirements / Qualifications

Mandatory

  • Tertiary Qualification or related qualification in Intensive care nursing or similar.
  • Current advanced life support certification.
  • Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Working rights within Australia.
  • Minimum five years’ experience in Critical Care.
  • CHS is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates and contingencies.

Highly Desirable

  • Demonstrated ability to deliver quality nursing care in accordance with the ANMC National Competency Standards for a Registered Nurse
  • Prior experience as a Nurse Liaison or member of an ICU outreach team will be highly desirable.
  • Experience providing leadership to a team and development of a service.
  • Fulfil the responsibilities of this role in alignment to the CHS Exceptional Care Framework and all other related frameworks.
  • Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
