Position Overview

The Discharge Liaison Nurse works in collaboration with the multidisciplinary team to co-ordinate discharge planning. They ensure the discharge plan is patient centred, adheres to best practice and facilitates safe, timely and sustainable transfer of care.

Duties

Under limited direction of the Discharge Liaison Team Leader (CCC) you will perform your role within the multidisciplinary team to co-ordinate discharges in a safe, appropriate, timely and sustainable pathway.

  1. Working independently and in conjunction with other health professionals to lead and coordinate safe, appropriate, and timely discharge of patients from inpatient units to achieve targeted date of discharge:
  2. Adopt an ‘early intervention’ approach and actively participate in family meetings to support patients and families through the discharge process ensuring patient centred care.
  3. Co-ordinate multi-disciplinary and handover meetings to discuss management plan and discharge destinations.
  4. Co-ordinate, within tight time frames, the safe discharge/ transfer of patients from the unit on day of discharge including organising the relevant discharge documentation. Maintain effective communication with the multi-disciplinary team throughout this process to ensure hospital discharge times are met.
  5. Develop and maintain effective networks with a range of internal/external stakeholders and service providers.
  6. Remain current in specialised knowledge, expertise and skills through continuing professional development and ongoing education.
  7. Support safe and evidence-based practice collaboratively through:
    1. Facilitation of evidence-based quality improvement activities that reflect the changing trends in service delivery and align with the National Safety and Quality Health Service (NSQHS) Standards.
    2. Support of policy change and new clinical initiatives to foster a dynamic workplace.
  8. Communicate effectively and appropriately (verbally and in writing) using consistent and transparent processes including the ability to communicate effectively with patients who have a cognitive impairment
  9. Provide leadership and promote a commitment to high quality customer service principles and practices as a role model for junior and new graduate staff within scope of practice.
  10. Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Position Requirements / Qualifications

Mandatory

  • Be registered as a Registered Nurse (Division 1) or be eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • The successful applicant will need to work Monday to Friday with potential for rotating weekend work, 76 hours per fortnight.
  • CHS is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates and contingencies.

  3. Highly effective communication and negotiation skills, including demonstrated ability to work effectively as a team member and to establish and maintain professional relationships with internal and external stakeholders to ensure appropriate continuity of care for consumers.
  4. Evidence of commitment to professional development, and participation in quality improvement and education activities.
  5. Demonstrates understanding of, and adherence to, safety and quality standards, work, health and safety (WH&S) and the positive patient experience. Displays behaviour consistent with CHS’s values.
