Position Overview
Under limited direction of the Diabetes Nurse Practitioner and the Clinical Nurse Educator (CNE) of the North Canberra Hospital Diabetes Nursing Team, the DE is responsible for providing holistic, client centred and safe care to clients with diabetes.
This involves providing individual and group appointments, collaborating with Endocrinologists, Diabetes Nurse Practitioner, Diabetes Nurse Educators, Dietitians, Nurses, Medical Officers, General Practitioners, Allied Health Professionals and people with diabetes and their support person to provide a team approach to care. This position also requires demonstrated excellent communication skills with all key stakeholders involved with the patient and to perform as a professional role model, mentor and clinical resource for other health care colleagues.
Duties
Under limited direction of the Nurse Practitioner Diabetes and the CNE CDE you will perform a co-ordinated and structured approach within the Diabetes Nursing Service and broader hospital. You will:
- Provide holistic, client centred and safe care to clients with diabetes throughout the care continuum.
- Provide advanced guidance and direction in teaching, preceptorship, mentoring and coaching to both registered and unregistered staff, undergraduate students, and other health staff, to achieve a high standard of clinical practice and client outcomes.
- Provide expert leadership and coordination in procedure development, professional standards, research, peer review, performance appraisal and continuous quality improvement.
- Represent the diabetes service on internal and external committees.
- Actively pursue own continuing education and professional development.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Position Requirements / Qualifications
Mandatory
- Completed or completing a Post Graduate Certificate in Diabetes Education and Management qualifications. Is an Australian Diabetes Educators Association (ADEA) member and has or is working towards becoming a Credentialed Diabetes Educator.
- Be registered or be eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- CHS is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates and contingencies.
Highly Desirable
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
- Fulfil the responsibilities of this role in alignment to the CHS Exceptional Care Framework and all other related frameworks.

What you Require (Key Selection Criteria)

- Demonstrated recent clinical experience within diabetes education and management for people with diabetes in the acute setting.
- Demonstrated clinical assessment skills, sound judgment and problem solving abilities, a proven ability to work autonomously, and in collaboration with a multi-disciplinary team.
- Demonstrated experience in communication skills, including the effective negotiation and conflict resolution, ability to liaise effectively with diverse external and internal stakeholders, and interact as part of a multi-disciplinary team.
- Demonstrated ability to initiate, implement, embed and evaluate quality improvement, education and research projects which contribute to improved knowledge, practice and care relevant to people with diabetes.
- Demonstrates understanding of, and adherence to, safety and quality standards, work, health and safety (WH&S) and the positive patient experience. Displays behaviour consistent with CHS’s values of reliable, progressive, respectful and kind.
