Position Overview
Clare Holland House provides Specialist Palliative Care Services throughout the ACT including Home-Based, Inpatient, and Outpatient settings along with outreach to Residential Aged Care Facilities (RACF) and North Canberra Hospital.
The Home-Based team provide Specialised Palliative care for patients in the community.
The team consists of Doctors, RNs, NP, and Allied Health (including Occupational Therapists, Physios, Social workers, and Spiritual Care supports).
The Home-Based Team Leader (TL) is responsible for the day-to-day service delivery by Specialist Palliative Care Nurses to our community patients and works Monday – Friday, 0700 – 1530, excluding public holidays.
You support the CNC, to make sure the team are safe, you ensure patients receive the care they need and the escalation when required.
Duties
One of the main roles of the HBPC TL is holding and answering the 24-hour line. This is the number given to our patients and their loved ones to call if they have any questions or concerns, this number is also called by staff who are out on the road and need guidance, and other healthcare professionals seeking advice or wishing to place a referral.
Under limited direction of the CNC, you will perform. The following duties:
- Receive handover from LINK and follow up on overnight concerns; run the morning huddle to update staff and flag issues.
- Allocate staff to daily snapboard, book new admissions based on urgency and capacity and support staff to start their visits early.
- Monitor Home-Based patients: run daily report on location, phase, and unscheduled visits.
- Coordinate with inpatient CNC for potential admissions; update families and arrange transport.
- Alongside the CNC, run weekly MDT for bereavement and patients of concern, support referrals to other services.
- Support new staff orientation and assist with clinical handovers and debriefs.
- Check drug count daily with TL and evening RN; hand over to evening shift.

Position Requirements / Qualifications
Mandatory
- Be registered or be eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Have a minimum of 3 years post grad experience in nursing.
- Have experience working in Palliative Care, with the understanding of the fundamentals of Palliative Care.
- CHS is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates and contingencies.
Highly Desirable
- Previous experience in leadership roles.
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
- Fulfil the responsibilities of this role in alignment to the CHS Exceptional Care Framework and all other related frameworks.
