Position overview

Patient Navigation Services include the Liaison and Navigation Service (LaNS) and the Paediatric Liaison and Navigation Service (PLaNS). These services provide liaison, navigation, care coordination and complex case management for people with complex health and psychosocial needs.

Patient Navigation Services improve consumer experiences and health outcomes through information sharing, integration of care across primary, secondary, and tertiary services, and coordination across health and community services. They focus on the provision of right care, at the right time, at the right place, by the most appropriate provider in partnership with the consumer and their family/care supports. This includes a focus on providing care outside the acute setting through collaboration with primary and community service providers and outreach to clients in the ACT community.

The service is comprised of a multidisciplinary team of nursing and allied health professionals that assist consumers and their families/supports to navigate the multiple services involved in their care. The service provides and supports patient-centred, trauma informed, strengths-based, integrated, and coordinated care. It works closely with primary, acute and community services, and across health and psychosocial services, to reduce the challenges consumers and their families/supports face when required to access multiple services, to ensure the highest quality of care.

The Patient Navigation Team Leader will provide clinical and project support and is a key leadership position for the team of Registered Nurses and Health Professionals providing services to consumers referred to Patient Navigation Services, their families, and their care supports. This position will work across both the adult LaNS and paediatric PLaNS.

Key Responsibilities

Specific responsibilities include, but are not limited to, the following:

  1. Be accountable for leading and coordinating the clinical care for consumers: providing leadership and using critical analysis, clinical judgement and applying evidence-based best practice to assessment and interventions provided by the team.
  2. Provide clinical care and leadership, including expert consultation and clinical decision-making in relation to consumers with presentations of significant health and psychosocial complexity with an understanding of the social determinants of health, and a focus on achieving enhanced health and wellbeing outcomes.
  3. Provide leadership and expertise relating to clinical governance, policy development, planning, evaluation, quality improvement, and research. Mentor and support professional development and competencies of colleagues and maintain own advanced level of knowledge through relevant opportunities in education and management.
  4. Coordinate and facilitate project documentation, processes, guidelines, reporting. Quality improvement, and research activities in relation to the continuity of shared care for consumers, including compliance with relevant legislation, policy, and regulatory frameworks.
  5. Manage resources, workflows, and issues, as well as establishing work priorities, developing strategies, and evaluating outcomes.
  6. Liaise and consult effectively and maintain key internal and external stakeholder relationships including government and non-government organisations.
  7. Lead a positive workplace culture through collegial professional relationships, collaboration, strong communication, and a proactive solution focus.
  8. Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications

Essential

  • Hold an undergraduate or postgraduate qualification in a relevant Allied Health profession (Social Work, Psychology or Occupational Therapy preferred) or relevant experience.
  • Be registered (or be eligible for registration) with Australian Health Practitioner Regulatory Agency (AHPRA) or Australian Association of Social Work (AASW).
  • Hold a current Driver’s License and be willing to travel to meet clients and stakeholders in the ACT community.

  • Substantial clinical experience in the acute, sub-acute, community and/or primary health setting of greater than 5 years (full-time work experience).

Desirable

  • Clinical experience within professional field working with people with complex health care needs.
  • Experience providing or coordinating care across acute, community and/or primary care settings.
  • Previous experience in clinical supervision and/or people management.
  • Experience working with individuals who experience mental illness/mental disorder and insight into their needs, experiences and outcomes when accessing health services.
  • Experience which provides insight into consumer perspectives of managing complex ongoing healthcare
  • Experience or knowledge of the CHS Exceptional Care Framework, and all other related frameworks.

Prior to commencement

Appointment to this position is conditional on being granted and retaining appropriate clearances:

  • Pre-employment National Police Check.
  • Current registration issued under the Working with Vulnerable People (Background Checking) Act 2011 is required.
  • Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.
  • Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.

  3. Demonstrated ability to liaise, consult, negotiate and communicate with a range of key stakeholders, service organisations, and colleagues, with high level communication skills including compliance of reports, analysis of data, and development of processes and pathways to improve service delivery, consumer engagement, and the provision of comprehensive care.
  4. Demonstrated commitment to professional development, active participation in mentorship and education of others, and proven leadership experience including the ability to provide strategic direction, contribute to change management in a healthcare environment, lead quality improvement research activities, and develop clinical governance systems and processes to optimise evidence-based practice.
  5. Demonstrates understanding of, and adherence to, safety and quality standards, work, health and safety (WH&S) and the positive patient experience. Displays behaviour consistent with CHS’s values of reliable, progressive, respectful and kind.
