Position overview
Rehabilitation, Aged and Community Services (RACS) is a vibrant and diverse Division within Canberra Health Services providing multidisciplinary rehab, aged and community-based care across a range of settings. This includes Canberra Hospital, University of Canberra Hospital, Community Health Centres and Village Creek Centre in Kambah. University of Canberra Hospital is Canberra’s first purpose-built rehabilitation hospital and supports people recovering from surgery, injury, or experiencing mental illness.
Key Responsibilities
Specific responsibilities include, but are not limited to, the following:
- Provide high level rehabilitation care consistently and competently to patients of varying complexity, including acting as a role model, a core clinical resource person, and a source of rehabilitation knowledge
- Coordinate the transition of patients accepted into the Rehabilitation Service.
- Assist in the overall discharge process for rehabilitation patients by attending Goal Setting meetings, facilitating Case Conferences, attending Family meetings and Discharge Planning meetings, supporting the MDT to improve performance against expected date of discharge, providing support and updates to patients, families and staff through the rehabilitation and discharge planning process and ensuring appropriate referrals are actioned in a timely manner.
- Provide timely and accurate reporting of activity and performance.
- Support professional development and education within the specialty by regular presentation and participation in area- specific activities such as in-service education, support and promotion of reflective practice and providing a leadership role within the Journal club
- Support research and safe evidence-based practice through supporting policy change quality improvement and new clinical initiatives in order to foster a dynamic workplace.
- Remain current in specialised knowledge, expertise and skills through continuing professional development and ongoing education
- Undertake other duties appropriate to this level of classification and within the approved scope of clinical practice which contribute to the operations of the organisation.

Qualifications
Essential
- Relevant tertiary qualifications (physiotherapist, social worker or occupation therapist) or equivalent in an Allied Health related field.
- Registered or be eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA) or for allied health professions not regulated by National Law be eligible for accreditation with the Australian Association of Social Workers.
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.

- Experience or knowledge of the CHS Exceptional Care Framework, and all other related frameworks.
Prior to commencement
Appointment to this position is conditional on being granted and retaining appropriate clearances:
- Pre-employment National Police Check.
- Current registration issued under the Working with Vulnerable People (Background Checking) Act 2011 is required.
- Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.
- Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.

- Proven comprehensive knowledge and experience in the provision and planning of care for rehabilitation patients, their families and carers.
- Demonstrated experience in effective resolution of complex problems and issues Including but not limited to work within a multi-disciplinary team and communicate effectively.
- Demonstrated clinical competence and specialised knowledge of rehabilitation care and recent experience in the acute and sub-acute hospital setting.
- Evidence of commitment to professional development, and participation in quality improvement activities, education activities, and resource management.
- Demonstrates understanding of, and adherence to, safety and quality standards, work, health and safety (WH&S) and the positive patient experience. Displays behaviour consistent with CHS’s values of reliable, progressive, respectful and kind.
