Position overview

Reporting to the Executive Director of Cancer and Ambulatory Support (CAS), the Program Director, Integrated Care is responsible for coordinating, driving and delivering work to implement a range of strategies to achieve CHS objectives in delivering enhanced integrated care to those experiencing chronic and complex conditions in the ACT. The position will work closely with a range of stakeholders internally and externally to CHS to ensure collaborative approaches to service delivery with a focus on improving health service efficiency and integration.

Key Responsibilities

Under limited direction you will play a key role in day-to-day human resource and project management. You will:

  1. Provide effective leadership and direction to ensure implementation of a range of strategies to achieve the objectives of integrated care and meet relevant budget initiatives and election commitments.
  2. Drive partnerships with staff and organisations to facilitate change management initiatives to achieve the CHS strategic objectives.
  3. Work collaboratively with multidisciplinary services to ensure high quality and sustainable integrated health services across the Territory.
  4. Assist in the development of key projects and detailed implementation plans ensuring these are managed within budget, time, and scope.
  5. Actively engage in the Health Infrastructure program, including service redesign and model of care development for the new community health hubs.
  6. Lead, coordinate and facilitate reporting on plans, progress, quality improvement and risk management.
  7. Undertaking other duties appropriate to this level of classification which contribute to the operation of the Division.

Qualifications and Experience

Essential

  • Tertiary qualifications (or equivalent) in a health or related discipline.
  • Current Driver’s License.
  • A minimum of 5 years’ experience in health-related project or management roles.

Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates, and contingencies.

  3. Proven experience in developing and managing systems for clinical governance and quality improvement, including knowledge of contemporary health care practices, and future directions and innovations in healthcare.
  4. Proven ability to manage financial, physical and human resources including within an infrastructure development setting.
  5. Demonstrates understanding of, and adherence to, safety and quality standards, Work Health and Safety and the positive patient experience. Displays’ behaviour consistent with CHS’s values of reliable, progressive, respectful, and kind.
