Position overview
Reporting to the Senior Director, Integrated Care, the Director of Strategic Initiatives is responsible for coordinating, driving and delivering complex, whole-of-system strategic initiatives that improve integration across acute, primary, community and social sectors in alignment with the Vision, Mission and Purpose of the Integrated Care Program:

You will support the operations, development, and continuous improvement of the Integrated Care Program including the Paediatric Liaison and Navigation Service (PLaNS), the Liaison and Navigation Service (LaNS), Priority Populations and other projects or service development initiatives. You will lead, coordinate, and contribute to the ongoing development of Integrated Care Program services and initiatives to ensure the evolution of sustainable and evidence-informed processes and programs to improve integration of care across the system.
As a senior officer and active member of the Integrated Care Management Team, you will also play a key role in supporting the effective operational leadership of the service, including fostering positive culture, growth, capability and wellbeing of individuals and teams.
The position works closely with a wide range of stakeholders both within CHS and across the broader health and community sectors. Key Responsibilities
Under the limited direction of the Senior Director, Integrated Care, you will:
- Provide effective strategic leadership and direction to ensure successful implementation of a range of initiatives and strategies to achieve the objectives of the Integrated Care Program.
- Plan, develop, lead and coordinate program priorities, service activities and deliverables for the Integrated Care Program using data-driven project management principles, stakeholder engagement and change management strategies to drive high-quality outcomes.
- Actively contribute to the leadership, management and development of the interdisciplinary team, including supporting individuals, team processes, and team culture across multiple service sites.
- Lead the development, promotion and implementation of projects, initiatives and change processes, including quality improvements activities, policies and program documentation.
- Liaise and collaborate with stakeholders internally in CHS as well as across health and community sectors to plan, coordinate and implement Integrated Care projects and initiatives that are person and family centred, strengths-based and trauma-informed.
- Undertake research, prepare high-level written material and provide expert strategic, operational and administrative advice, including internal briefs, Ministerial briefings, data analysis, reporting and evaluation to inform program performance and improvement.
- Represent the Integrated Care Program in internal, external and multi-agency forums, fostering strong partnerships and promoting integrated care principles and engaging collaborators to drive whole-of-system integration.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications and Experience
Specific qualifications and experience include, but are not limited to, the following.
Essential
- Hold a current Driver’s License and be willing to travel within the ACT and surrounds.
- A minimum of 5 years’ experience in health-related project or health management roles.
Desirable
- Tertiary qualifications (or equivalent) in a health or related discipline.
- Experience in a health management or leadership role.
- Experience or knowledge of the CHS Exceptional Care Framework, and all other related frameworks.
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.

Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates, and contingencies.
