Canberra Health Services (CHS) is focussed on the delivery of high quality, effective, person centred care. It provides acute, sub-acute, primary and community‐based health services, to the Australian Capital Territory (ACT) and surrounding region.

CHS Diabetes Service is a multidisciplinary team comprising medical, nursing and allied health professionals. The service provides a coordinated and integrated service between the Canberra Hospital and community health centres within the ACT. Services include:

  • Diabetes care and treatment for adolescents and adults with Type 1 and Type 2 diabetes
  • Women with Type 1 and Type 2 diabetes planning pregnancy and during pregnancy
  • Women with Gestational diabetes

The successful candidate will be responsible for the provision of clinical assessment, treatment, supervision and evaluation of nutrition care to inpatients and outpatients who have diabetes. This involves providing individual and group nutrition appointments collaborating with Endocrinologists, Diabetes Nurse Educators, Dietitians, Nurses and Midwives, General Practitioners and consumers to provide a team approach to care for people with diabetes.

Key Responsibilities

Specific responsibilities include, but are not limited to, the following:

  1. With support, provide individual and group nutrition education using appropriate assessments and interventions to clients with diabetes, using established evidenced based practice.
  2. Liaise with clients, carers and other service providers, as required.
  3. Complete clinical and administrative data collection and evaluation to standard.
  4. Participate in education programs (staff and consumers) and contribute to teaching and training across a range of clinical areas.
  5. Contribute to the multidisciplinary team approach and service activities including the implementation of best practice and evidenced based models.
  6. Participate in implementation of clinical governance activity, quality improvement projects, research programs, and health promotion areas relevant to service.
  7. Participate in supervision, continuing professional development and performance management.
  8. Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications

Specific responsibilities include, but are not limited to, the following.

Essential

  • Hold a degree or post graduate qualification in Nutrition and Dietetics.
  • Eligible for membership of Dietitians Australia and eligible to hold Accredited Practicing Dietitian (APD) credential.
  • Hold a current driver’s licence.

Appointment to this position is conditional on being granted and retaining appropriate clearances:

  • Pre-employment National Police Check.
  • Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.
  • Current registration issued under the Working with Vulnerable People (Background Checking) Act 2011 is required.
  • Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.

CHS is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates, and contingencies.
