​​​​The Pharmacy sits within the Division of Medical Services (EDMS) which includes the Physician Training Office, Medical Officer Support, Credentialing, Employment and Training Unit (MOSCETU), GP Liaison Unit (GPLU), Pathology, Pharmacy, Medical Imaging and Library Services. The Canberra Health Services (CHS) Pharmacy Department have a dynamic, talented team of approximately 100 staff, including: pharmacists, technicians, couriers, and administration staff. The department provides a range of clinical services to inpatients and outpatients including several specialised services.

​​Canberra Hospital is the only paediatric tertiary teaching hospital for the Australian Capital Territory and surrounding NSW region, serving a population of over half a million. The Division of Women, Youth & Children provides a broad range of primary, secondary and tertiary healthcare services. The paediatric services, co-located within the Centenary Hospital for Women, Youth & Children, can cater for inpatients, has an active medical and surgical day stay and clinical investigation unit, paediatric HITH services and a busy and growing outpatient department. There is a tertiary accredited Level 6 Neonatal Intensive Care Unit, and the hospital also offers subspecialty paediatric care in areas of Paediatric Respiratory Medicine, Paediatric Endocrinology and Diabetes and Paediatric Surgery. The Paediatric Surgical Unit performs a wide range of paediatric and neonatal surgery. The Department of Obstetrics and Gynaecology at Canberra Hospital provides tertiary level obstetrics and gynaecological services to the ACT and surrounding regions. The Centenary Hospital for Women and Children has more than 3,700 births per year and is a level 6 referral centre for high-risk pregnancies for the region.

​​The position holder will provide a range of pharmacy services to the Division of Women, Youth and Children to promote quality use of medicines that is safe and cost-effective to patients within Canberra Health Services (CHS). The position holder will provide clinical input under limited supervision of the Lead Pharmacist and provide a range of pharmacy services on a rostered basis. This position will also require maintaining some general pharmacist skills to contribute to the after-hours pharmacist roster at Canberra Hospital.

Key Responsibilities

Specific responsibilities include but are not limited to the following:

Under limited direction of the Lead Pharmacist Women, Youth & Children and Deputy Directors of Pharmacy you will work closely with a team of dedicated and skilled technicians, pharmacists, and other healthcare practitioners, to deliver a safe, efficacious, and patient centred service. The responsibility of this position is to provide clinical and operational pharmacy services across CHS within the specified clinical and operational areas and may include pharmacist support to off-site facilities. All positions will include a component of operational commitment as part of their work plan.

  1. Provide advanced level clinical pharmacy services to patients under the care of the Division of Women, Youth and Children, including participation in multidisciplinary ward rounds and clinical meetings. At the PH2 level this will be within a defined scope of practice, seeking guidance for complex or unfamiliar clinical situations, consistent with the Transition Level Advanced Pharmacy Practice competencies. At the PH3 level this will be at a higher level of autonomy and complexity, independently managing complex medication-related issues, including total parenteral nutrition, and providing expert advice to the multidisciplinary team, consistent with the Consolidation Level Advanced Pharmacy Practice competencies.
  2. Provide specialist advice regarding medicines used in neonates, infants, children, adolescents and pregnant or breastfeeding patients, therapeutic drug monitoring, medicines optimisation and patient/family-centred education, including expertise on weight-based and body surface area dosing, neonatal pharmacokinetics, extemporaneous formulations and methods of administration of medicines to this patient group. At the PH3 level, this includes contributing to digital medication safety work on medication builds, infusion concentrations, infusion pump libraries, and electronic medication systems optimisation.
  3. Dispensing and supply of medicines for inpatients, outpatients and for patients being discharged from the hospital services.
  4. Participate in activities to assist CHS to meet relevant National Safety and Quality Health Service Standards and Clinical Care Standards. This includes developing and contributing to medication safety initiatives, accreditation activities, publications, policy and procedure reviews and quality improvement projects. At the PH3 level, lead on these activities to improve professional practice, implement organisational priorities and improve patient outcomes, including contributing to organisation working groups and committees relevant to the role.
  5. Provide supervision, support and education to pharmacy students, pharmacy interns, less experienced pharmacists, and pharmacy technicians. At the PH3 level this includes providing mentoring and development of others within the team, contributing to workforce capability and standards of practice, being a ClinCAT evaluator and may include managing junior members of the team.
  6. Participate in education, training and research activities within the multidisciplinary team and apply evidence-based practice to support patient care. At the PH3 level, this includes designing and evaluating structured educational activities and contributing to research.
  7. Maintain generalist pharmacist skills in other areas to provide a range of pharmacy services on weekends, public holidays and after hours as rostered, including participating in an On-Call roster.
  8. Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Competencies

For a level 2 pharmacist position increment (PH2) - able to demonstrate competency against the National Standards for Advanced Pharmacy Practice at the Transition Level.

For a level 3 pharmacist position increment (PH3) - able to demonstrate competency against the National Standards for Advanced Pharmacy Practice at the Consolidation Level, which may include a post graduate qualification at diploma level or above.

And to uphold ethical standards, demonstrate the professional behaviours reasonably expected of a registered health professional and strive to improve quality and safety within the health system.

To communicate and work effectively with professional colleagues, patients (which includes carers, guardians, and families), other clients as well as members of the general public.

Domain 3: Medicines management and patient care

Apply their expertise to promote wellness or work in direct consultation with patients or in collaboration with other health professionals to improve health. Including participation in public health and health promotion activities, the assessment of patients to develop a patient-centered medication management plan and the provision of medicines (prescription and non-prescription) and compounded products.

Domain 4: Leadership and management

Demonstrate self-leadership as well as their leadership role in sharing a vision for the future and promoting the use of initiative and innovation to respond to change and drive progress. This includes applying management skills to undertake organisational and business planning, including the management of finances, human and other resources, the professional environment, and the service delivery arrangements.

Domain 5: Education and research

To contribute to the education of professional colleagues. Including the ability to analyse and synthesise information from medical and pharmaceutical literature, identify and respond to gaps in the evidence-base by conducting research, share research findings and apply evidence in practice.

Qualifications

  • Be registered or eligible for registration as a Pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA).

  • Experience of working professionally as a registered pharmacist in an Australian hospital environment (or in a country where reciprocal arrangements apply with the Australian Pharmaceutical Council Competency Stream Skills Assessment method).
  • The successful applicant will be appointed to the relevant level 2 or 3 pharmacist position increment based on their qualifications and experience:
  • For a level 2 pharmacist position increment – minimum of 3 years’ full time equivalent experience as a registered pharmacist in a hospital setting (as previously defined) or completion of the AdPha accredited 2 year full time equivalent foundation residency program, including experience of working in hospital based Women, Youth & Children areas.
  • For a level 3 pharmacist position increment – minimum of 4 years’ full time equivalent experience as a registered pharmacist in a hospital setting (as previously defined), including significant experience of working in Women, Youth & Children areas.

Desirable

  • Experience or knowledge of the CHS Exceptional Care Framework, and all other related frameworks.
  • Strong organisational skills with a high degree of personal drive and integrity.
  • Developed or emerging leadership skills and motivation with the ability to participate in the department's quality improvement projects, educational programs, and research activities.

Demonstrated clinical pharmacy knowledge and understanding of medicine principles relevant to Women, Youth & Children services in the hospital setting, including obstetrics, gynaecology, neonatology, paediatric medicine, paediatric surgery and adolescent health, consistent with the relevant level of Advanced Pharmacy Practice competencies. This includes providing advice on therapy choice, dose optimisation, therapeutic drug monitoring, and other issues relating to pharmacotherapy.
