​​​​The Pharmacy sits within the Division of Medical Services (EDMS) which includes the Physician Training Office, Medical Officer Support, Credentialing, Employment and Training Unit (MOSCETU), GP Liaison Unit (GPLU), Pathology, Pharmacy, Medical Imaging and Library Services. The Canberra Health Services (CHS) Pharmacy Department have a dynamic, talented team of approximately 100 staff, including: pharmacists, technicians, couriers, and administration staff. The department provides a range of clinical services to inpatients and outpatients including several specialised services.

​​A new Surgical Procedures, Interventional Radiology and Emergency Centre has just been opened which will transform the acute services delivered at the Canberra Hospital in the near future. This new centre will deliver additional Emergency Department treatment spaces, theatres, additional intensive care unit (ICU) beds, as well as including new paediatric ICU beds.

​​This position will provide clinical pharmacy services remotely across a range of clinical areas. ​

Key Responsibilities

Specific responsibilities include but are not limited to the following:

  1. Undertake clinical pharmacist duties within scope of practice, ensuring the delivery of high-quality person and family centred care that is safe and evidence based through remote service delivery models. At the PH2 level this will be within a defined scope of practice, seeking guidance for complex or unfamiliar clinical situations, consistent with the Transition Level Advanced Pharmacy Practice competencies. At the PH3 level this will be at a higher level of autonomy and complexity, independently managing complex medication-related issues within a defined area of practice and providing expert advice to the multidisciplinary team, consistent with the Consolidation Level Advanced Pharmacy Practice competencies.
    1. Review and verify medication orders for dispensing
    2. Provide clinical pharmacy services for patients where suitable information can be obtained remotely
    3. Participate in the remote surgical pre-admission clinic to support pre-operative medication reviews (depending on role).
    4. Respond to medicines information queries from the multidisciplinary team
    5. Perform other clinical pharmacy roles suitable for remote delivery.
  2. Counsel patients about their medications, in order to permit patients to achieve better understanding, acceptance of and concordance with their medication regimens; to optimise patients' therapeutic outcomes.
  3. Contribute to operational pharmacist duties if on-site visits are undertaken.
  4. Participate in the pharmacy quality improvement projects, medication safety and clinical governance activities and implementation of practice improvements. At the PH3 level, lead on these activities to improve professional practice, implement organisational priorities and improve patient outcomes. This includes contributing to organisation working groups and committees relevant to the rotation where meetings are held virtually.
  5. Provide remote supervision, support and education to pharmacy students, pharmacy interns, less experienced pharmacists, and pharmacy technicians. At the PH3 level this includes providing mentoring and development of others within the team, contributing to workforce capability and standards of practice, being a remote ClinCAT evaluator and may include managing junior members of the team.
  6. Participate in education, training and research activities and apply evidence-based practice to support patient care. At the PH3 level, this includes designing and evaluating structured educational activities and contributing to research.
  7. Provide pharmacy services on weekends, public holidays and after hours as rostered, which may include participating in a remote On-Call roster.
  8. Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Competencies

For a level 2 pharmacist position increment (PH2) - able to demonstrate competency against the National Standards for Advanced Pharmacy Practice at the Transition Level.

For a level 3 pharmacist position increment (PH3) - able to demonstrate competency against the National Standards for Advanced Pharmacy Practice at the Consolidation Level, which may include a post graduate qualification at diploma level or above.

The competencies defined within the National Competency Standards Framework for Pharmacists in Australia1 include the following 5 Domains:

Domain 1: Professionalism and ethics

To comply with legislative standards, including a legal responsibility to work within their competence, and commit to life-long learning and professional development to maintain and build competence. And to uphold ethical standards, demonstrate the professional behaviours reasonably expected of a registered health professional and strive to improve quality and safety within the health system.

Domain 2: Communication and collaboration

To communicate and work effectively with professional colleagues, patients (which includes carers, guardians, and families), other clients as well as members of the general public.

Domain 3: Medicines management and patient care

Apply their expertise to promote wellness or work in direct consultation with patients or in collaboration with other health professionals to improve health. Including participation in public health and health promotion activities, the assessment of patients to develop a patient-centered medication management plan and the provision of medicines (prescription and non-prescription) and compounded products.

Domain 4: Leadership and management

Demonstrate self-leadership as well as their leadership role in sharing a vision for the future and promoting the use of initiative and innovation to respond to change and drive progress. This includes applying management skills to undertake organisational and business planning, including the management of finances, human and other resources, the professional environment, and the service delivery arrangements.

Domain 5: Education and research

To contribute to the education of professional colleagues. Including the ability to analyse and synthesise information from medical and pharmaceutical literature, identify and respond to gaps in the evidence-base by conducting research, share research findings and apply evidence in practice.

Qualifications

Essential

  • Be registered or eligible for registration as a Pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA).

Essential

  • Experience of working professionally as a registered pharmacist in an Australian hospital environment (or in a country where reciprocal arrangements apply with the Australian Pharmaceutical Council Competency Stream Skills Assessment method).
  • The successful applicant will be appointed to the relevant level 2 or 3 pharmacist position increment based on their qualifications and experience:
  • For a level 2 pharmacist position increment – minimum of 3 years’ full time equivalent experience as a registered pharmacist in a hospital setting (as previously defined) or completion of a 2-year full time equivalent AdPha accredited foundation resident training program.
  • For a level 3 pharmacist position increment – minimum of 4 years’ full time equivalent experience as a registered pharmacist in a hospital setting (as previously defined) or completion of an AdPha accredited registrar training program in a relevant specialty area.

Desirable

  • Experience or knowledge of the CHS Exceptional Care Framework, and all other related frameworks.
  • Strong organisational skills with a high degree of personal drive and integrity.
  • Developed or emerging leadership skills and motivation with the ability to participate in the department's quality improvement projects, educational programs, and research activities.

Prior to commencement

Appointment to this position is conditional on being granted and retaining appropriate clearances:

  • Pre-employment National Police Check.
  • Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.

Demonstrated clinical pharmacy knowledge, judgement and skills consistent with the relevant level of Advanced Pharmacy Practice competencies, sufficient to provide pharmaceutical care to patients in a tertiary hospital setting across a variety of clinical specialties, including, but not exclusive to, medicine, surgery, women's health, paediatrics, aged care, mental health, drug and alcohol, haematology/oncology, critical care, emergency care and pharmacy operational areas, with the ability to deliver clinical pharmacy services effectively through remote models of care.
  2. High level interpersonal and communication skills, including the ability to consult, negotiate and liaise effectively with patients and with colleagues in the multidisciplinary setting, including effective communication and relationship building in a remote service environment.
  3. Demonstrated ability to provide professional supervision to junior pharmacists, graduates, students and support staff, with evidence of increasing leadership and mentoring capability, a commitment to continuing professional development and contribution to education and training activities.
  4. Demonstrated leadership and management capabilities consistent with the Transition or Consolidation Level Advanced Pharmacy Practice competencies, including contribution to or leading service improvement, implementation of change and the development and management of others.
  5. Demonstrated contribution to or leadership of quality improvement programs, policy and procedures, medication safety initiatives, research and accreditation activities.
