Position overview
The Transitional Therapy and Care Program (TTCP) provides up to 12 weeks of goal focused therapy and care services for older persons within their home or at the TTCP residential unit. The TTCP aims to maximise a person’s recovery and functional independence following a hospitalisation and prevent premature admission to an aged care facility. The TTCP Allied Health Team is based at the University of Canberra Hospital, Bruce. It comprises of Physiotherapists, Occupational Therapists, Dietitians, Social Workers, a Speech Pathologist and Allied Health Assistants. The TTCP program is delivered in collaboration with a Non-Government Organisation (TTCP Service Provider) which delivers the programs support services and runs the TTCP residential unit.
The TTCP Clinical Lead position provides leadership in clinical service delivery. The Clinical Lead is responsible for leading timely and effective referral management and rehabilitation coordination for consumers who are admitted to TTCP, with a greater focus on clients with complex needs. The successful candidate will work closely with the TTCP Manager, TTCP Liaison Officers, TTCP service provider, and the TTCP multi-disciplinary team to ensure high quality service delivery and evidence-based practices in line with organisational and national standards. They will also work collaboratively with inpatient teams from public and private hospitals, the Aged Care Assessment Organisation, the Canberra Health Services Integrated Operations Centre, and with their inpatient Allied Health, Nursing and Medical management colleagues, to optimise positive client outcomes and flow.
Key Responsibilities
Under limited direction of the TTCP Manager, you will work collaboratively to:
- Provide high level effective clinical leadership, expertise and coordination across a range of clinical areas relating to aged care and rehabilitation and within an interdisciplinary model.
- Provide leadership and clinical expertise to support efficient and effective management of the TTCP referral and admission process, including liaison with relevant stakeholders.
- Provide leadership in clinical coordination to ensure high quality and safe service delivery for clients accessing TTCP, with a focus on supporting staff to manage complex clients.
- Provide leadership within the TTCP team to optimise access to the service, including support to facilitate patient flow and discharge from the TTCP service.
- Establish and maintain professional relationships with the interprofessional team, including internal and external stakeholders, using effective interpersonal and communication skills.
- Facilitate the delivery of exceptional care through clinical governance activities, implementing contemporary evidence-based practices, facilitating quality improvement activities, innovation, research, data collection and evaluation.
- Initiate, contribute to, and provide leadership for clinical standards, procedure and guideline development, change management, and service planning in collaboration with clinicians, senior roles, TTCP manager and CHIC Executive.
- Work in partnership with the TTCP Manager and the TTCP service provider to provide high quality services, including meeting Australian Government requirements as outlined in the Transition Care Program Guidelines, and conformance with National Safety and Quality Health Service (NSQHS) and Aged Care Quality Standards.
- Participate in continuing professional development, and supervision and/or training to staff and students.
- Undertake other duties appropriate to this level of classification as directed, within the approved scope of clinical practice ensuring the delivery of high-quality person and family centred, safe and high-quality patient care.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Qualifications
Essential
- Undergraduate or postgraduate qualification in a relevant Allied Health Profession. (e.g. Physiotherapy, Occupational Therapy, Social Work, Dietitian or Speech Pathologist)
- Registered or eligible for registration with the Australian Health Practitioner Regulation Agency (AHPRA), or relevant Allied health Professional Organisation.
- Hold a current driver licence.
Desirable
- Any other relevant post-graduate qualifications will be highly regarded.
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
Experience
Essential
- Minimum of 5 years’ experience working as an Allied Health Professional in a relevant clinical area. Proven high level clinical experience.
Desirable
- Extensive experience working as an allied health professional in rehabilitation and aged care.
- Experience or knowledge of the CHS Exceptional Care Framework and all other related frameworks.
Prior to commencement
Appointment to this position is conditional on being granted and retaining appropriate clearances:
- Pre-employment National Police Check.
- Current registration issued under the Working with Vulnerable People (Background Checking) Act 2011 is required.
- Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.
- Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.
