We have two Community Specialist Palliative Care teams.
Home Based Palliative Care
This team provides an in home palliative care service to residents of the ACT. We’re focused on supporting that you’re able to stay at home as long as possible.
Palliative Aged Care Specialists (PEACE)
The PEACE team provides Specialist palliative consultancy services into all Residential Aged care homes (RACH) in the ACT.
Who we care for
We care for people from the ACT with a valid referral.
How to access this service
You need a medical referral for the Inpatient Unit at Clare Holland House. Referrers, are General Practitioner, Medical Specialist, or Medical practitioner. The referral will be assessed based on admission criteria and clinical needs.
What to expect
Home based palliative care
If you’re part of our Home Based Palliative Care program, we provide assessment and care planning from the multidisciplinary team based on your individual circumstances and goals of care.
Our team focuses on your comfort and dignity and maintaining your quality of life through your palliative care journey.
You will have access to phone support 24hrs a day.
PEACE
Assessment and advice from a specialist palliative care practitioner to assist your GP in managing your symptoms and end of life care needs.
This service operates Monday to Friday business hours.
