About this service
The Co-occurring Needs Program (CNP) is a new initiative that is providing direct assessment with MHJHADS clients experiencing co-occurring mental ill health and substance use challenges. Currently, the program provides consultation liaison support to community MHJHADS teams through assessment and treatment recommendation to ensure integrated care meets the consumer's needs. CNP provides short-term assessment and treatment planning, including brief interventions, harm minimisation and relapse prevention planning suited to diverse populations.
CNP also aims to upskill the MHJHADS clinicians to work in an integrated space, providing education, resources and clinical support.
Who we care for
The Co-occurring Needs Program is available for those with ongoing MHJHADS clinical contact above the age of 12. Ongoing clinical contact with an assigned MHJHADS team is required for CNP involvement due to the short-term nature of involvement. Referrals are only available by MHJHADS clinical teams.
What to expect
A referral will be made with your consent by an MHJHADS clinician following a conversation around how substance use/mental health challenges impact your recovery and wellbeing. An appointment will be scheduled where you are encouraged to invite family, friends or supporters to discuss your current challenges and be open to support options. The CNP team will ask some questions which will help direct treatment options. A comprehensive treatment plan will be co-created and shared for how to best meet your needs. Our service is voluntary; however, we aim to provide a trauma-informed and recovery-oriented plan to suit your goals.

Where we're located
We are flexible on where we provide assessment and treatment and are guided by the referrer and person with co-occurring needs. The assessment can occur at a community health centre or in the person’s home if suitable. If the referred person is incarcerated or admitted to a mental health or alcohol and drug service, we will coordinate with relevant stakeholders to ensure assessment and treatment can be facilitated.
