Our Assertive Community Outreach Service (ACOS) treats people who have severe and long-lasting signs of psychosis.

About this service

We provide long-term clinical management, treatment, and care in the community for adults who have severe and long-lasting signs of psychosis and complex needs.

Who we care for

We provide long-term, intensive clinical management for you if you:

  • Are 18 years old or older
  • Have severe and long-lasting signs of psychosis
  • Have had a poor response to medication and treatment
  • Have complex needs such as other mental health issues, disability, homelessness, drug and alcohol use
  • Are at risk of harming yourself or others, or at risk of being harmed by others.

Psychiatric appointments are free if you have a Medicare Card or Asylum Seeker Card.

How to access this service

Your clinical manager or Community Recovery Service can refer you to this service.

What to expect

Our team may visit you at your home or organise to meet with you at your local community health centre. We will do a mental health assessment and provide treatment and care.

We will work with you and people close to you, if you agree, (such as family, carers, friends, support providers, guardians and financial managers, and other medical professionals) to provide treatment and care.

What to bring to your appointment

  • Medicare card
  • Any letters or correspondence from your GP
  • Health Care Card and/or concession card (if you have one)
  • Private health insurance card (if applicable/if you want to use it)
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
  • Previous x-ray films, scans, ultrasounds or any other test results or reports

We will usually visit you at home or organise to see you at a community health centre.
