Aged Care Assessment Team (ACAT)
Assess the needs of frail older people and help them to access support.
Clinical Psychology (for older people with cognitive difficulties or people recovering from brain injury or illness)
Provides assessment, diagnosis and treatment of psychological and emotional problems.
Falls and Falls Injury Prevention Program
Helps people who have experienced falls or are looking for ways to minimise their risk of falling.
General Geriatric Clinic (Comprehensive Geriatric Assessment)
Comprehensive geriatric assessment of older people with multiple and complex medical issues.
General Medicine at Canberra Hospital
Helps people with undiagnosed medical conditions or multiple active medical problems.
Geriatric Rapid Acute Care Evaluation (GRACE) Service
Provides clinical care to Canberrans in residential aged care facilities to better manage their conditions, preventing a trip to hospital.
Memory Assessment Service
Provides early diagnosis, treatment, advice and referral for people under 70 with early memory changes or possible dementia.
Older Persons Mental Health Community Teams
Assessment, care planning and interventions to people who are 65 years old or older who are experiencing a moderate to severe mental illness.
Parkinson's Disease Clinic for Older People
Offers comprehensive assessment of the older people who have features of Parkinsonism.
Rapid Assessment of the Deteriorating Aged at Risk (RADAR)
Visits older people in their own home to do assessments with the aim of keep older people safe at home.
Residential Aged Care Liaison Nurse
Provides support as you move from hospital into a residential aged care facility.
Social Work - Aged Care Inpatient Service
An inpatient social work service that supports people over 80 years old with a sudden or age-related illness.
Transitional Therapy and Care Program (TTCP)
The Transitional Therapy and Care Program (TTCP) is a short-term, goal-oriented program that aims to improve the independence of older people after being in hospital.
We also provide:
- an acute inpatient service at Canberra Hospital, and
- a Subacute Geriatric Unit (for people who are not severely ill) at University of Canberra Hospital.
