Central Health Intake (CHI) is the primary gateway for individuals accessing community-based services and specialist outpatient care through Canberra Health Services. Our dedicated nursing team plays a critical role in delivering timely, coordinated, and person-centred care by managing intake, assessment, and referral pathways for a range of services. Operating within a unified management structure, CHI nurses provide expert clinical support to both the Community Health Intake and Central Outpatient Intake administration teams. This co-located and collaborative environment promotes efficiency and ensures a smooth healthcare journey for consumers and clinicians across the Canberra Health Services network.
Key responsibilities
Under limited direction of the CHI Clinical Nurse Consultant, your responsibilities include, but are not limited to, the following:
- Assist intake, assessment, and referral processes to support equitable access to care.
- Apply clinical knowledge within your scope of practice to referral screening to ensure consumers are matched with appropriate clinical services.
- Liaise with internal teams and external providers to support timely, efficient, and coordinated service delivery.
- Communicate clearly and professionally via phone, written, and digital channels; maintain accurate records to support service delivery.
- Adhere to policies and professional standards; engage in ongoing education to support clinical competencies and operational development.
- Engage in clinical decisions and support quality improvement, education, and accreditation activities.
- Promote safe practice and comply with WHS, infection control, equity, and diversity principles.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.
Competencies

- Demonstrated ability to assess complex patient referrals and ensure appropriate service engagement within professional scope of practice.

Qualifications
Specific responsibilities include, but are not limited to, the following.
Essential
- Tertiary Qualification in Nursing
- Be registered or eligible for registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency (AHPRA).
Desirable
- CHS is at the forefront of digital transformation in healthcare, implementing a territory-wide Digital Health Record.
- This role requires strong computer literacy to complete documentation and confidently use digital systems. Ongoing engagement with system updates and changes is essential to maintain proficiency.
Experience
Specific responsibilities include, but are not limited to, the following.
Essential
- Minimum one (1) year (1710 hours) undergraduate and/or post-graduate experience in an acute or sub-acute care setting
Desirable
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
- Fulfil the responsibilities of this role in alignment to the CHS Exceptional Care Framework and all other related frameworks.
Prior to commencement
- Appointment to this position is conditional on being granted and retaining appropriate clearances.
- Pre-employment National Police Check.
- Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.
