We provide treatment and care for people 18-65 years of age with serious and enduring mental illness or dysfunction.
About this service
Our services focus on recovery and include:
- medical reviews
- clinical management
- assistance with accessing the National Disability Insurance Scheme and other community supports
- involvement with specialized clinics for people prescribed Clozapine
Who we care for
Our services are available for people aged 18-65 years, who have been referred to us by the Access Mental Health Team or inpatient mental health teams.
You or your loved one may see us voluntarily, or you or your loved one may be seeing us under a Psychiatric Treatment Order.
We offer these services for free if you have a Medicare Card or an Asylum Seeker Card.
How to access this service
You will need to contact the Access Mental Health Team to access our service or speak with your current mental health team if you think this service is right for you.
What to expect
You will be encouraged to attend your local community health centre regularly to see your clinical manager and treating doctor.
You will be invited to develop a care plan as part of your recovery. Our aim is to help you recover enough to be cared for by your GP - your care plan will include this.
Our team will confirm appointments with you by letter and telephone.
What to bring to your appointment
- Medicare card
- Any letters or correspondence from your GP
- Health Care Card and/or concession card (if you have one)
- Private health insurance card (if applicable/if you want to use it)
- List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
